Publication: Samsun İli 112 Çalışanlarında Tükenmişlik, İş Doyumu ve İlişkili Faktörler
Abstract
Amaç: Bu çalışmada Samsun İl Sağlık Müdürlüğü İl Ambulans Servisi Başhekimliğine bağlı 112 acil sağlık hizmetlerinde görev yapan sağlık çalışanlarında tükenmişlik, iş doyumu ve ilişkili faktörlerin belirlenmesi amaçlanmıştır. Gereç ve Yöntem: Kesitsel tipteki araştırmanın evrenini, Samsun ilinde 15/07/2020-15/09/2020 tarihleri arasında İl Sağlık Müdürlüğü İl Ambulans Servisi Başhekimliğine bağlı olarak hizmet veren Komuta Kontrol Merkezi ve Acil Sağlık Hizmetleri istasyonlarında görev yapan 617 kişi oluşturdu. Evren ortalamasının temsil edilebilmesi için gereken minimum örneklem sayısı 311 kişi olarak hesaplandı. Araştırma 337 kişi ile tamamlandı. Çalışanlara; sosyodemografik özellikleri içeren veri formu (26 soru), Maslach Tükenmişlik Ölçeği (MTÖ-22) ve Minnesota İş Doyumu Ölçeği (MİDÖ-20) uygulandı. Bulgular: Çalışmaya katılanlar %40,7'si erkek, %59,3'ü kadın olmak üzere toplam 337 kişiydi. Yaş ortalaması 33,1±5,9 (22-54) yıldı. Katılımcıların ölçek ortanca puanları duygusal tükenmişlik için 29,0 (9-45), duyarsızlaşma için 10,0 (5-24), kişisel başarı için 29,0 (8-40) ve genel doyum için 3,0 (1,1-5,0) idi. Duygusal tükenmişlik kadınlarda ve lise mezunu olanlarda anlamlı daha yüksekti (p<0,05). Doktorların, mobbing ve şiddete uğrayanların duyarsızlaşması daha yüksekti ve anlamlıydı (p<0,05). Çocuk sahibi olanların ve 10 yıldan fazla çalışanların kişisel başarı puanları anlamlı daha düşüktü (p<0,05). Genel doyum ise; sigara kullanmayanlarda ve koşulları yeterli bulanlarda anlamlı daha yüksekti (p<0,05). Duygusal tükenmişlik ve duyarsızlaşma ile genel doyum arasında anlamlı ve negatif ilişki, kişisel başarıda azalma ile genel doyum arasında ise anlamlı ve pozitif ilişki saptandı (p<0,05). Duygusal tükenmişliği kadın olmak 2,3 kat, lise mezunu olmak 5,0 kat artırırken, duyarsızlaşmayı ekonomik durumun kötü olması 2,4 kat, yeterli güvenliğin olmamasının 1.7 kat artırdığı bulundu. Kişisel başarıda azalmanın ise, yaşın ilerlemesiyle azaldığı ve meslekteki yılın artmasıyla 1.6 kat arttığı bulundu. İş doyumu açısından değerlendirildiğinde ise, kadın olmanın 1.7 kat, meslekte ilerleyen yılların 1.1 kat ve sigara kullanımının 2.1 kat iş doyumunu azalttığı bulundu (p<0,05). Sonuç: Çalışmamızda, 112 çalışanlarında; duygusal tükenmişlik ve duyarsızlaşma ile genel doyum arasında anlamlı ve negatif ilişki, kişisel başarıda azalma ile genel doyum arasında ise anlamlı ve pozitif ilişki saptanmıştır. Ancak, 112 çalışanlarında tükenmişlik ve iş doyumunun nedensellik ilişkisinin ortaya konulabilmesi için daha kapsamlı araştırmalara ihtiyaç duyulmaktadır. Material and method: The population of the cross-sectional study consisted of 617 people working in the Command Control Center and Emergency Health Services stations serving under the Provincial Health Directorate Provincial Ambulance Service Chief Physician between 15/07/2020 and 15/09/2020 in Samsun. The minimum number of samples required to represent the population mean was calculated as 311 people. The research was completed with 337 people. To the employees; A data form containing sociodemographic characteristics (26 questions), Maslach Burnout Scale (BTS-22) and Minnesota Job Satisfaction Scale (MIDS-20) were applied. Results: The participants in the study were 337 people, 40.7% male and 59.3% female. The mean age was 33.1±5.9 (22-54) years. Participants' median scores on the scale were 29.0 (9-45) for emotional burnout, 10.0 (5-24) for depersonalization, 29.0 (8-40) for personal achievement, and 3.0 (1.1-5.0) for general satisfaction. Emotional burnout was significantly higher in women and high school graduates (p<0.05). Depersonalization of doctors, victims of mobbing and violence was higher and significant (p<0.05). The personal achievement scores of those who had children and those who worked for more than 10 years were significantly lower (p<0.05). General satisfaction is; It was significantly higher in non-smokers and those who found the conditions sufficient (p<0.05). A significant and negative relationship was found between emotional exhaustion and depersonalization and general satisfaction, and a significant and positive relationship was found between decreased personal achievement and general satisfaction (p<0.05). It was found that while being a woman increased emotional exhaustion 2.3 times, being a high school graduate 5.0 times, depersonalization increased 2.4 times due to bad economic situation and 1.7 times due to lack of sufficient security. It was found that the decrease in personal achievement decreased with increasing age and increased 1.6 times with increasing years in the profession. When evaluated in terms of job satisfaction, it was found that being a woman decreased job satisfaction 1.7 times, years in the profession 1.1 times, and smoking 2.1 times (p<0.05). Conclusions: In our study, 112 employees; There was a significant and negative relationship between emotional exhaustion and depersonalization and general satisfaction, and a significant and positive relationship between decreased personal achievement and general satisfaction.
