Publication: Multipl Skleroz Hastalarında Acil Klinik ve Paraklinik Durumlar
Abstract
Giriş ve amaç: Multipl skleroz (MS) enflamasyon, demiyelinizasyon ve akson hasarı ile karakterize otoimmün bir santral sinir sistemi (SSS) hastalığıdır. Myelin kılıflar, oligodendrositler ve daha az oranda akson ve sinir hücresi hasarlanır. Hastalık sıklıkla genç yetişkinlerde ortaya çıkar. Prevalansı coğrafi özelliklere bağlı olarak 100.000 de 2 ile 200 arasında değişmektedir. Bu çalışma MS tanısı ile takip edilen hastalarda acil servise başvuru nedenlerini ve sonuçlarını ayrıntılı olarak analiz etmek için yapılmıştır. Gereç ve yöntem: Çalışmaya Ondokuz Mayıs Üniversitesi Tıp Fakültesi Nöroloji Ananbilim Dalı MS Birimde takip edilen MS hastaları dahil edilmiştir. Hasta verilerine İmed veri tabanından ulaşılmıştır. Bulgular: çalışmaya 2100 MS hastası dahil edilmiştir. Hastaların %68,6'sı kadın hastalardır. Hastaların büyük bir kısmı relapsing remitting MS hastalarıdır. Acil servise başvuru yapan hasta sayısı 190 (%9) olarak bulunmuştur. Acile başvuran 190 hastanın toplam 304 acil servis başvurusu bulunmaktadır. En sık acile başvuru nedeni enfeksiyonlar ve epileptik nöbetlerdir. Bu durumu genel durum bozukluğu, ataklar ve senkop kliniği izlemektedir. En sık enfeksiyon sebebi solunum yolu ve idrar yolu enfeksiyonlarıdır. Atak şikayeti ile acile başvuran hasta sayısı oldukça düşüktür. Hastaların 26'sında (%1,2) mortalite geliştiği sonucuna varılmıştır. Sonuç: Multipl skleroz hastaları acil servis başvuru için hastalık ile direk ilişkili atak gibi nedenlerden daha çok enfeksiyonlar ve epileptik nöbetler gibi şikayetleri için başvuru yapmaktadırlar.
: Multiple sclerosis (MS) is an autoimmune central nervous system (CNS) disease characterized by inflammation, demyelination and axon damage. Myelin sheaths, oligodendrocytes and, to a lesser extent, axons and nerve cells are damaged. The disease often occurs in young adults. Its prevalence varies between 2 and 200 per 100,000, depending on geographical features. This study was conducted to analyze in detail the reasons and results of admission to the emergency department in patients followed up with the diagnosis of MS. Materials and methods: MS patients followed in the MS Unit of Ondokuz Mayıs University Faculty of Medicine, Department of Neurology were included in the study. Patient data were obtained from the Imed database. Results: 2100 MS patients were included in the study. 68.6% of the patients are female patients. Majority of the patients are relapsing remitting MS patients. The number of patients who applied to the emergency department was 190 (9%). There were 304 emergency service applications from 190 patients who applied to the emergency department. The most common causes of emergency admission are infections and epileptic seizures. This situation is followed by general condition disorder, attacks and syncope clinic. The most common causes of infection are respiratory and urinary tract infections. The number of patients who applied to the emergency department with the complaint of attacks is very low. It was concluded that 26 (1.2%) of the patients developed mortality. Conclusion: Multiple sclerosis patients apply to the emergency department for complaints such as infections and epileptic seizures, rather than for reasons such as attacks directly related to the disease.
