Abstract
Amaç: Bu çalışmanın amacı; aktif ankilozan spondilit (AS) hastalarında Anti- TNF tedavi ve global postural reedukasyon (GPR) egzersiz kombinasyon tedavisinin ağrı, hastalık aktivitesi, mobilite, yorgunluk, uyku kalitesi, depresyon, yaşam kalitesi ve fonksiyonel kapasiteye olan etkisinin araştırılmasıdır. Gereç ve yöntem: Modifiye New York ve/veya ASAS aksiyel spondiloartropati kriterlerini karşılayan 60 AS tanılı hasta çalışmaya alındı. Hastalar 3 gruba ayrıldı. Birinci gruba Anti-TNF tedavi + GPR egzersiz programı verildi. 2. gruba Anti-TNF tedavi + konvansiyonel egzersiz programı verildi. 3. gruptaki hastaların mevcut tedavileri (NSAİİ ve/veya SLZ) değiştirilmeksizin sadece konvansiyonel egzersiz programı verildi. Klinik değerlendirmede hastalık aktivitesi için Bath Ankilozan Spondilit Hastalık Aktivite İndeksi (BASDAI), fonksiyonellik için Bath Ankilozan Spondilit Fonksiyonel İndeksi (BASFI), mobilite için lomber schober, göğüs ekspansiyonu, el-parmak zemin mesafesi, yorgunluk değerlendirmesi için Yorgunluğu Çok Yönlü Değerlendirme Anketi (MAF), yaşam kalitesi değerlendirmesi için Kısa Form 36 (KF-36), uyku kalitesi değerlendirmesi için Pittsburgh uyku kalitesi indeksi (PUKİ), depresyon değerlendirmesi için Beck Depresyon Ölçeği (BDÖ) kullanıldı. Ayrıca fonksiyonel performansı değerlendirmek için 6 dakika yürüme testi uygulandı. Tüm hastalar tedavi öncesi ve 3. ayda değerlendirildi. Bulgular: Hastaların demografik özellikleri kıyaslandığında, gruplar arası anlamlı fark bulunmadı. Grup içi değerlendirmede; Anti-TNF+ egzersiz tedavisi alan her iki grupta tedavi öncesine göre tedavi sonrası 3. ayda tüm değerlendirme parametrelerinde istatistiksel olarak anlamlı gelişme olduğu gözlendi. Gruplar arasında değerlendirme parametrelerinin tedavi öncesi ve sonrası skor farklarının karşılaştırmasında; Anti- TNF+GPR egzersiz tedavisi ile ağrı, mobilite ve fonksiyonel performansta Anti TNF+ konvansiyonel egzersiz tedavisine göre istatistiksel olarak daha anlamlı gelişme gözlendi. Hastalık aktivitesi, fonksiyonellik, yorgunluk, yaşam kalitesi, uyku kalitesi ve depresyon değerlendirmesinde konvansiyonel egzersizlerle benzer etki gözlendi. Sonuç: Egzersiz ve Anti-TNF tedavi kombinasyonundan oluşan her iki tedavi de ağrıyı, hastalık aktivitesini, fonksiyonel performansı, yorgunluğu, uyku kalitesini, yaşam kalitesini ve depresyonu iyileştirmede efektiftir. Aktif AS hastalarında GPR egzersiz yöntemi ile ağrı, mobilite ve fonksiyonel performansta daha belirgin iyileşme gösterilmesi nedeni ile, motive hastalar bu egzersiz yöntemini uygulamaları için cesaretlendirilmelidir. Aim: The purpose of this study was to investigate the effects of combination therapy with global postural reeducation exercise (GPR) and Anti-TNF treatments on pain, disease activity, mobility, fatigue, sleep quality, depression, quality of life and functional capacity in patients with active ankylosing spondylitis (AS). Materials and Methods: 60 AS patients who meet the criteria of Modified New York and / or ASAS axial spondyloarthropathy were included in the study. Patients were divided into 3 groups. The first group was given Anti-TNF therapy plus GPR exercise program. The 2nd group was given Anti-TNF and conventional exercise therapy. The 3rd group was given conventional exercise program along with their existing treatments (NSAIDs and/or SLZ). Following inventories are used for clinical evaluation: for disease activity - Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), for functionality - Bath Ankylosing Spondylitis Functional Index (BASFI), for mobility - lumbar Schober, chest expansion, , hand-finger to floor distance, for fatigue - fatigue Multidimensional Assessment Questionnaire (MAF), for quality of life - (SF-36), for sleep quality - Pittsburgh sleep quality index (PSQI), for depression - Beck depression Inventory (BDI), for functional performance - 6-minute walking test. All patients were evaluated before treatment and at 3 months. Results: The demographic characteristics of the patients were compared and there was no significant difference between the groups. The improvements in all parameters were better in both groups receiving exercise and Anti-TNF therapy than in the control group after treatment compared with baseline. The Anti-TNF + GPR exercise therapy resulted in greater improvements than the Anti TNF+conventional exercise therapy in pain, mobility and functional performance parameters. Conclusion: Both exercises with Anti-TNF therapy are efficient in improving pain, disease activity, functional performance, fatigue, sleep quality, quality of life, and depression. Since the improvements in pain, mobility and functional performance were greater in the active AS patients who performed the exercise according to GPR method, motivated patients should be encouraged to perform this exercise program.
