Publication: Türkiye Sağlık Sektöründe Kamu Özel İşbirliği ve Şehir Hastaneleri
Abstract
Amaç: Bu çalışma ile Türk sağlık sektöründe kamu özel işbirliği (KÖİ) uygulamalarının incelenmesi, irdelenmesi ve Türkiye'de son yıllarda açılmakta olan şehir hastaneleri konseptinin daha iyi anlaşılması amaçlanmaktadır. Materyal ve Metot: Nitel veri toplama teknikleri ile ele alınan bu araştırmada, yarı yapılandırılmış görüşme tekniği kullanılmıştır. Türkiye'de sağlık sektöründe yeni hizmet vermeye başlayan şehir hastanelerinin yöneticilerine yönelik, şehir hastanelerinin analizi için açık uçlu sorulardan oluşmuş yarı yapılandırılmış bir görüşme formu hazırlanmıştır. Görüşme formunda 12 adet açık uçlu soru mevcuttur. Sorular, literatür taraması yapılarak ve uzman görüşü alınarak hazırlanmıştır. Çalışma Ankara, Mersin, Adana, Yozgat ve Manisa şehir hastanelerinde gerçekleştirilmiştir. Başhekimler ile yapılan görüşmeler ses kaydına alınmış, ardından çalışma amacına uygun olarak düzenlenip yazıya aktarılmıştır. Bulgular: Araştırmada şehir hastanelerinin çalışanların üstünde fazla iş yüküne sebebiyet verdiği, hastaların hastaneye ulaşılabilirliğine yönelik çözümlerin arttırıldığı, mahkum ve tutuklulara daha güvenlikli alanlarda hizmet verildiği, hasta bekleme sürelerine yönelik çalışmaların yapıldığı, hastanelerin sağlık turizmi konusunda önemli potansiyel taşıdığı, Sağlık Bakanlığının hastane üzerindeki denetleme rolünün arttığı, diğer kamu hastanelerine göre daha üstün kaliteli sağlık hizmeti alındığı, nitelikli sağlık hizmeti sunumuna yeni bir boyut kazandırdığı bulgularına ulaşılmaktadır. Sonuç: Şehir hastanelerinde sağlık personel sayısının istenilen düzeyin altında kaldığı, verilen tıbbi ve destek hizmet kalitesinin arttığı, hastaların hastaneye ulaşımı için daha fazla yol kat ettiği, güvenlik önlemlerinin arttığı ve hasta bekleme süresinin kısaldığı sonuçlarına ulaşılmıştır.Anahtar Kelimeler: Kamu özel işbirliği; sağlık sektörü; şehir hastaneleri Anahtar Kelimeler: Kamu özel işbirliği; sağlık sektörü; şehir hastaneleri
Purpose: This paper aims to analyze and explore the implementations of Public-Private Partnership (PPP) in Turkey's health sector and also generate a better understanding of Turkey's city hospitals concept. Materials and Method: In this research which has been made with the qualitative data gathering techniques, semi-structured interview technique has been applied. A semi-structured interview form consisting of open-ended questions was prepared for the managers of city hospitals who started to provide new services in Turkey's health sector. There are 12 open-ended questions in the interview form. The questions were prepared both with a literature review and expert opinion. Our study was carried out in Ankara, Mersin, Adana, Yozgat and Manisa city hospitals. The interviews with the head doctors were recorded and then arranged and transcribed in accordance with the purpose of the study. Findings: In this research, it is observed that county hospitals cause more workload on the employees, however solutions for the accessibility of patients to the hospital are increased, services which given to the prisoners and detainees are provided in safer areas, researches are carried out for patient waiting periods, hospitals have significant potential in health tourism, the role of the Ministry of Health in the control of the hospital has increased, compared to other public hospitals more quality health care has been obtained and finally it is observed that these county hospitals have brought a new dimension in the provision of qualified health care. Conclusion: Through the present findings we conclude that the number of medical staff is below the desired number, on the other hand the quality of medical and aid service has increased, in regard of accessibility of patients to the hospitals much progress has been made, additional security measures have been applied and patient's waiting time has decreased.
