Sınıf Iı Malokluzyon Tedavisinde Sabit Fonksiyonel Bir Apareyin Dentofasiyal Etkilerinin İncelenmesi

Abstract

ÖZETSINIF II MALOKLUZYON TEDAVİSİNDE SABİT FONKSİYONEL BİR APAREYİN DENTOFASİYAL ETKİLERİNİN İNCELENMESİFuat TOKALAK, Doktora TeziOndokuz Mayıs Üniversitesi, Samsun, Haziran 2012Bu çalışmanın amacı iki farklı büyüme gelişim dönemindeki hasta grubuna uygulanan herbst apareyinin iskeletsel, dişsel ve fasiyal etkilerinin incelenmesidir.Araştırmamızda her biri 12 bireyden oluşan iki farklı uygulama grubu oluşturuldu. Birinci uygulama grubu yaş ortalaması 12,8 ± 1,1 yıl olan pubertal büyüme atılım dönemindeki bireylerden, ikinci uygulama grubu yaş ortalaması 14,54 ± 1,17 yıl olan geç adölesan ve erken erişkin dönem bireylerden oluşturuldu. Bireyler mandibular geriliğe bağlı Sınıf II Bölüm 1 malokluzyona ve daimi dentisyona sahip olgular arasından seçildi.Tedavi başlangıcında (T1) bireylerden lateral sefalometrik filmler, ağız dışı ve içi fotoğraflar, ortodontik alçı modeller alındı. Elde edilen modeller üzerinde krom-kobalt döküm splintler hazırlandı. Herbst apareyi laboratuar aşamalarından sonra ağız içerisine yerleştirildi. Ortalama 6 aylık tedavi süresi sonunda aparey çıkartılarak tedavi başlangıcında alınan kayıtlar yinelendi (T2). Klasik sefalometrik ölçümler ve Pancherz'in sagital okluzal analiz yöntemi ile sefalometrik filmler değerlendirildi. Gruplar içerisindeki değişimler eşleştirilmiş t testi ile gruplar arasındaki farklılıklar ise bağımsız t testi ile değerlendirildi.Tedavi sonunda her iki grupta da sınıf II molar ilişki ve artmış overjet başarılı bir şekilde tedavi edildi. Efektif mandibular uzunluk iki grupta da artarken profilkonveksitesi iki grupta da azaldı. Herbst apareyinin her iki büyüme gelişim dönemi arasında tedavi etkinliği açısından anlamlı bir fark bulunamadı.

ABSTRACTEVALUATION OF DENTOFACIAL EFFECTS OF A FIXED FUNCTIONAL APPLIANCE IN TREATMENT OF CLASS II DIVISION I MALOCLUSSIONFuat TOKALAK, Ph.D. ThesisOndokuz Mayıs University, Samsun, June 2012Aim of this study was to evaluate skeletal, dental and facial effects of Herbst appliance applied to two different patient groups with respect to their growth and development stages.Two groups, each containing 12 patients, were enrolled in the study. Mean pretreatment age was 12.8 ± 1.1 years for the first group (adolescents) and 14.54 ± 1.17 years for the second group (late adolescents and early adults). Selected patients had Class II division I malocclusion with retrognathic mandibles relative to other cranio-facial structures and all of them were in permanent dentition stage.Lateral sephalometric films, extra oral and intra oral photographs and orthodontic models were taken at the beginning of treatment (T1). After Cr-Co cast splints were prepared on laboratory models appliances were applied to the patients. The average treatment period was 6 months and all pretreatment records were renewed at the end of treatment (T2). Sephalometric films were evaluated both with classic sephalometric measurements and Pancherz?s sagital occlusal analysis method. The differences within and between groups were evaluated by Paired t test and Independent t test respectively.At the end of treatment both class II molar relationship and increased overjet were successfully treated. Effective mandibular length was increased while profile convexity was reduced in both groups. The herbst appliance showed equal treatment results on two different growth and development stages.
