Abstract
Amaç: Bu tez çalışmasının amacı, maksiller posterior bölgede implant uygulaması için yeterli kemiğin bulunmadığı belirlenen hastalarda; Lökosit ve Trombositten Zengin Fibrin (L-TZF) uygulaması ile sinüs tabanı yükseltme işlemi yapılarak, konik ışınlı bilgisayarlı tomografi (KIBT) görüntülerinde kemik kazancının değerlendirilmesidir. Ayrıca, implant stabilitesi, KIBT görüntülerinde gri değerler ve hastalarda oluşan postoperatif semptomlar incelenmiştir. Materyal ve Metot: Çalışmaya ≤5mm vertikal kemik yüksekliğine sahip, aynı seansta sinüs tabanı yükseltmesi için L-TZF ve implant uygulanan 18 hasta dahil edildi. KIBT görüntülerinde operasyon sonrası elde edilen kemik kazancı ve gri değer ölçümleri yapıldı. İmplant stabilitesi cerrahi sırasında ve postoperatif 6.ayda rezonans frekans analizi (RFA) ile değerlendirildi. Ayrıca hastalar ağrı, şişlik gibi postoperatif semptomlar açısından da izlendi. Bulgular: Yaş ortalaması 50,8 olan 18 hastada toplam 35 implant, ortalama 18 ay takip edilmiştir ve implant kaybı olmamıştır. Operayon sonrası 6. ayda KIBT görüntülerinde ortalama 4 mm kemik kazancı sağlandığı görülmüştür. Ortalama ISQ değerleri operasyon sonrası 58,61±10,73, 6.ayda 65,86±7,01 olarak ölçülmüştür. KIBT ile belirlenen gri değerlerle sekonder stabilite arasında zayıf pozitif bir ilişki olduğu görülmüştür. Ağrı, şişlik gibi postoperatif semptomlarda zamanla azalma olduğu belirlenmiştir. Sonuç: Atrofik maksiller posterior bölgede L-TZF ile sinüs tabanı yükseltilmesi ve eş zamanlı implant uygulaması yeni kemik oluşumunu sağlamaktadır ve başarılı bir yöntem olarak düşünülebilir. Aim: The aim of this study was to determine, radiographically,the relevance of Platelet Rich Fibrin as the sole filling material during a sinüs floor augmentation with implantation in patients who had atrophic posterior maxilla. Additionally, implant stability, gray values on CBCT images and post-operative symptoms in patients were examined. Material and Method: Eighteen patients with vertical bone height of ≤5mm who underwent L-PRF and implant for sinus floor augentation in the same session were included in the study. Post-operative bone gain and gray value measurements were performed on CBCT images. Implant stability was evaluated by resonance frequency analysis (RFA) during surgery and at 6 months post-operatively. Patients were also evaluated for post-operative symptoms such as pain and swelling. Results: A total of 35 implants in 18 patients with a mean age of 50.8, were followed for an average of 18 months and no implant loss was occured. On the 6th post-operative month, an average of 4 mm bone gain was achieved in CBCT images. The mean ISQ values were 58.61 ± 10.73 immediately after surgery and 65.86 ± 7.01 6 month after surgery. There was a weak positive correlation between gray values determined by CBCT and secondary stability. Post-operative symptoms such as pain and swelling reduced over time. Conclusion: The application of sinus floor augmentation with L-PRF can provide new bone formation and might be considered as a successfull method.
