Total Diz Artroplastisi Sonuçlarımız

Abstract

Bu çalışmada total diz artroplastisinin tarihçesi, diz eklemi anatomi ve biyomekaniği, diz protezlerinin özellikleri, preoperatif hazırlık, cerrahi teknik ve komplikasyonlarda bahsedilmiştir.Ondokuz Mayıs Üniversitesi Tıp Fakültesi ve Araştırma Hastanesi Ortopedi ve Travmatoloji Kliniğinde Ocak 2007 - Aralık 2008 tarihleri arasında 144 hastanın 187 dizine total diz artroplastisi uygulanmıştır. Yeterli takibi olan 40 hastanın 53 dizi bu çalışmaya dahil edilmiştir. Hastalarımızın yaş ortalaması 61,82 olup, yaşları 34 ile 91 arasındadır. Ortalama takip süremiz 43,95 aydır.Diz skoru 75-100 arasında, ortalama 88,09 olarak saptandı. Diz skoru baz alınarak yapılan değerlendirmede dizlerin 39'unda (%73,58) mükemmel, 14'ünde (%26,41) iyi sonuç elde edilmiştir.Diz fonksiyon skoru 35-100 arasında, ortalama 74,62 olarak bulundu. Diz fonksiyonel skoru baz alınarak yapılan değerlendirmede dizlerin 17'sinde (%32,08) mükemmel, 16'sında (%30,19) iyi, 9'unda (%16,98) orta ve 11'inde (%20,75) kötü sonuç elde edilmiştir.Tüm hastalar dikkate alındığında çok iyi dizlerin oranı %73,58'dir. Bu değer pek çok çalışmadaki sonuçlara yakın olarak bulunmuştur. Doğru hasta seçimi, hastaya uygun protez seçimi, iyi bir cerrahi deneyim ile birlikte daha iyi sonuçlara ulaşılacağı açıktır.

In this study the history of total knee arthroplasty, knee joint anatomy and biomechanics of the knee prosthesis characteristics, preoperative preparation, surgical technique, and complications are discussed.Ondokuz Mayıs University School of Medicine and Research Hospital, Clinic of Orthopedics and Traumatology, January 2007 - December 2008, 144 patients of 187 total knee arthroplasty was applied to the directory. 53 series of 40 patients with adequate follow-up were included in this study. The mean age was 61.82, among the age of 34 to 91. The mean follow-time 43.95 months.Knee score of 75-100, the average was 88.09. The assessment made on the basis of knee scores in 39 knees (73.58%), good in 14 (26.41%) good results were obtained.Knee function score of 35-100, the average was 74.62. The assessment made on the basis of the functional knee score, 17 knees (32.08%), good in 16 (30.19%), fair in 9 (16.98%) moderate and 11 (20.75%) poor results were obtained.All patients were given the good and very good 73.58% of the knees is. This value was found to be close to many study results. The correct patient selection, patient selection of the appropriate prosthesis, with a good surgical experience is better than the results achieved.
