Abstract
Iskemik durumların miyokardiyal dokudaki serbest L-Carnitin düzeyinin düşmesi ile birlikte olduğu invitro olarak gösterilmiş ve L-Carnitin'in çeşitli mekanizmalarla iskemik miyokardı koruduğu öne sürülmüştür. Egzersiz uyafımlı stabil anjinapektorisi olan 14 hastada günde Igr oral Carnitin tedavisi 1 ay boyunca verilerek Bruce protokolü ile L-Carnitin tedavisi öncesi, 1 ay sonra ve ilaç bırakıldıktan 1 hafta sonrası, egzersiz EKG incelemeleri yapılarak L-Carnitin 'in iskemi parametreleri üzerindeki etkisinin araştırılması amaçlandı. Dört haftalık L-Carnitin tedavisi ile ortalama maksimal egzersiz süresi 5.660.62 dk'dan 6.470.79 dk'ya uzadı (p0.05). Maksimum iş yükü 6.570.53 METS'den 7.710.74 METS'e yükseldi (p0.05). Maksumum ST depresyonu, 1 mm 'lik ST depresyonu başlangıç zamanı, makimal kalp hızı ve basınç - nabız çarpım ürününde anlamlı bir değişiklik olmadı.
Effects of L-Carnitin on Stable Angina Pectoris It has been shown that ischemic conditions are associated with reduction of free L-Carnitine levels in myocardial tissue and suggested that L-Carnitine may protect ischemic miyocardium by various mechanisms. In this study it was aimed to investigate the effects of L-Carnitine on ischemia parameters in patients with exercise induced stable angina pectoris. Fourteen patients were given 1 g L-Carnitine once a day orally for one month. Exercise ECG was performed before and at the end of therapy and repeated one week later without any therapy. After the treatment period of 4 weeks total exercise durationwas increased from 5.660.62 minutes to 6.470.79 (p<0.05) Maximal work-load was increased from 6.57±0.5 METS to 7.710.74 (p<0.05). Maximal ST depression, time to. initiation ofST depression of 1 mm, maximal heart rate and double product were not altered significantly.
