Abstract
ÖZET AMAÇ : Nonproliferatif ve proliferatif diabetik retinopatili gözlerde argon laser fotokoagülasyonun etkinliğini araştırmak. GEREÇ VE YÖNTEM : Ondokuz Mayıs Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı Retina Biriminde 1980-2003 tarihleri arasında takip edilen 392 hastanın 771 gözü çalışma kapsamına alındı. Hastaların 231'i kadın 161'i erkek idi. Çalışmaya alınan gözlerin 611'inde ağır, çok ağır nonproliferatif diabetik retinopati, 160'ında proliferatif diabetik retinopati bulguları mevcuttu. Klinik olarak anlamlı makula ödemi 183 gözde mevcuttu. Beş yüz altmış göze panretinal fotokoagülasyon, 211 göze panretinal ile kombine fokal yada grid fotokoagülasyon tedavisi uygulandı. Hastaların tedavi öncesi ve sonrası görme keskinlikleri ve gelişen komplikasyonlar kaydedildi. BULGULAR : Hastaların yaş ortalaması 60,4 ± 9,37 (28-90), diabet süresi ortalama 13, 1± 6,34 (1-33) yıl, ortalama takip süreleri 3,2 ± 3,10 (1-23) yıl olarak tespit edildi. Çalışmaya alınan gözlerde, panretinal fotokoagülasyon sonrası % 10,6 görmede artma, % 52,3 görmede stabilizasyon ve % 37,1 görmede azalma bulundu. Makula ödemi olan grupta ise, % 13,4 görmede artma, % 51,5 görmede stabilizasyon, %35,1 görmede azalma tespit edildi. Takipler sırasında vitre içi hemoraji (%4) ve neovasküler glokom (%1,9) en sık görülen komplikasyonlardı. Proliferatif diabetik retinopati olan 160 gözün 143 'ünde (%89,4) tedavi sonrası neovaskülarizasyon geriledi. SONUÇ: Laser fotokoagülasyon diabetik retinopatinin tedavisinde etkin bir tedavi yöntemidir. Bununla birlikte bazı hastalarda tedaviye rağmen görmede azalma ve retinopatide ilerleme önlenememektedir.
ABSTRACT PURPOSE : To investigate the effect of argon laser photocoagulation in eyes with nonproliferative and proliferative diabetic retinopathy. MATERIAL AND METHODS : Seven hundred seventy one eyes of 392 patients followed in the retina unit of Ondokuzmayis University Faculty of Medicine, Ophthalmology Department in the period between 1980-2003 were included to the study. Two hundreds thirty one patients were females and the remaining 161 patients were males. Six hundreds eleven of the eyes included to the study had severe, very severe nonproliferative diabetic retinopathy whereas proliferative retinopathy findings were exist in 160 eyes. Clinically significant macular edema was found in 183 eyes. Panretinal photocoagulation therapy was applied to 560 eyes, on the other hand, combination of panretinal and focal or grid photocoagulation treatment was applied to 211 eyes. The visual acuities before and after the treatment and the complications developed were reported. FINDINGS : The average age of the patients was 60,4 ± 9,37 (28-90), the average period of diabetes was 13,1 ± 6,34 (1-33) and the average period of follow up was 3,2 ± 3,10 (1-23) years. In the eyes included to the study, the visual acuities increased in 10,6% of the eyes, remained stable in 52,3% of the eyes and decreased in 37,1% of the eyes following panretinal photocoagulation treatment. On the other hand in the group of macular edema, visual acuity was increased in 13,4%, remained stable in 51,5% and decreased in 35,1% of the eyes after the treatment. The most common complications found were vitreous hemorrhage (4%) and neo vascular glaucoma (1,9%). Neovascularization regressed in 143 of 160 eyes (89,4)%) with proliferative diabetic retinopathy after the treatment. RESULTS : Argon laser photocoagulation is an effective method in the treatment of diabetic retinopathy. However, inspite of the treatment in some cases visual decrease and deterioration of the retinopathy could not be prevented. VI
