Abstract
Giriş ve Amaç: Çalışmamızda Multipl Skleroz (MS) hastalarında seksüel ve üriner disfonksiyon görülme sıklığı ile klinik, demografik ile görüntüleme bulguları ile ilişkisinin belirlenebilmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmamıza Ondokuz Mayıs Üniversitesi Multipl Skleroz Polikliniği'nde takipli olup MS tanısı olan 161 hasta alındı. Çalışmaya alınan MS hastalarına çalışmayı yürüten hekim tarafından nörolojik muayenesi yapıldı. Hastalara, Genişletilmiş Özürlülük Durum Ölçeği (EDSS-Expanded Disability Status Scale), Multipl Skleroz Yakınlık ve Cinsellik Sorgulaması (Multiple Sclerosis Intimacy and Sexuality Questionnaire-19,MSISQ-19), Uluslararası Erektil Disfonksiyon Değerlendirme Formu (International Index of Erectile Function-IIEF), Kadın Cinsel Fonksiyon İndeksi (Female Sexual Function index-FSFI), Beck Depresyon Ölçeği (Beck Depression Inventory – BDI), SEAPI (Yaşam Kalite Endeksi), OABv8( Aşırı Aktif Mesane Sorgulama Formu) uygulandı. Kullanılan ölçekler, çalışmaya katılanlara muayeneyi gerçekleştiren nöroloji uzmanı ve psikolog tarafından yapıldı. Hastaların klinik ve demografik verileri MS polikliniğinde kullanılan İMED MS veri tabanından alındı. Bulgular: Çalışmamızda hastaların 103'ü kadın (%63,9), 58 erkek (%36,1) idi. Hastalarının 88'inde (%54,6) MSISQ19 skoruna göre cinsel fonksiyon bozukluğu mevcuttu. Bu hastaların %63,6'sı kadın, %36,4 ü erkekti. Cinsel fonksiyon bozukluğu olan hastaların özürlülüğü daha fazla, hastalık süresi daha uzun saptandı. Üriner inkontinans %64,1 oranında görüldü. Üriner inkontinans ve hiperaktif mesane semptomları olan hastalarda spinal lezyon daha sık görüldü. CIS klinik fenomeninde üriner ve cinsel disfonksiyon daha az görülmüştür. Sonuç: Çalışmamız MS hastalarında cinsel ve üriner disfonksiyonun sorgulandığı klinik kökenli bir çalışmadır.Çalışmamızda geniş bir değerlendirme parametresi kullanılmıştır. Daha büyük örnekler SD ile MS arasındaki ilişkiyi daha iyi anlamamıza yardımcı olabilir. Bununla birlikte, seksüel ve üriner disfonksiyon gözden kaçan oldukça sık görülen semptomlardır ve bu hastaların yaşam kalitesini olumsuz etkiler. Bu sorunun farkındalığının artırılması, bu hastalar için uygun tedavi ve yönetimler getirmek ve yaşam kalitesini arttırmak için yardımcı olabilir.
Introduction and objective: In our study, the incidence of sexual and urinary dysfunction in patients with Multiple Sclerosis (MS) and clinical, demographic and imaging findings relationship. Material and methods : Our study included 161 patients with MS who were admitted to the Multiple Sclerosis Polyclinic of Ondokuz Mayıs University. Neurological examination was performed by the physician who was working for the MS patients who were taken into the study. We applied EDSS (Expanded Disability Status Scale), MSISQ-19) (Multiple Sclerosis Intimacy and Sexuality Questionnaire-19), IIEF (International Index of Erectile Function), FSFI (Female Sexual Function index), BDI (Beck Depression Inventory), SEAPI , OABv8 (Evaluating the 8-item overactive bladder questionnaire) to all patients. The scales used were made by the neurology specialist and psychologist who performed the examination to the participants. Clinical and demographic data of the patients were taken from the IMED MS database used in the MS policlinic. Results: In our study, 103 patients were female (63,9%) and 58 were male (36,1%). 88 patients (54.6%) had sexual dysfunction according to MSISQ19 score. 63.6% of these patients were female, 36.4% were male. Patients with cognitive impairment had more disability and longer duration of illness. Urinary incontinence was seen in 64.1% of cases. Spinal lesions were more common in patients with urinary incontinence and hyperactive bladder symptoms. Urinary and sexual dysfunction are less common in the CIS clinical phenomenon. Conclusion: Our study is a clinical study of sexual and urinary dysfunction in MS patients. A wide range of evaluation parameters used. Larger examples do not understand the relationship between SD and MS better might help. However, sexual and urinary dysfunction escaping are quite common symptoms and affect the quality of life of these patients. It awareness of the problem, appropriate treatment and management for these patients and life can help to improve the quality.
