Abstract
Bu çalışmada, Ondokuz Mayıs Üniversitesi Tıp Fakültesi Gastroenteroloji Bilim Dalı Endoskopi Ünitesi' ne gastroduodenal yakınmalarla gelen hastalardan alınan biyopsi örneklerinden Helicobacter pylori izolasyonu ve antibiyotik duyarlılık testlerinin yapılması amaçlanmıştır. Bu amaçla Ocak 2012 - Haziran 2012 tarihleri arasında hastanemiz Gastroenteroloji Bilim Dalı Endoskopi Ünitesine başvuran, dispeptik yakınmalı, endoskopik tanıları antral gastrit, eroziv gastrit, pangastrit, duodenit, reflü özefajit ve peptik ülser olan hastalardan alınan antral biyopsi örnekleri incelemeye alınmıştır.Çalışmaya dahil edilen hastaların mide antrumundan 3'er biyopsi örneği alınmış, biyopsi örneklerinin gram boyamaları, üreaz testleri ve kültürleri yapılarak H. pylori varlığı araştırılmıştır. Kültür için vankomisin, trimetoprim, cefsulodin ve amfoterisin B eklenmiş ( Helicobacter pylori Selective Supplement, SR0147E), % 7 defibrine at kanlı Brain Hearth Infüsion agar kullanılmıştır. Ekim yapılan besiyerleri anaerobik jarlar içerisinde, GENbox kitleri ile sağlanan mikroaeorofilik ortamda 37 °C'de 3-10 gün inkübe edilmiştir. İzole edilen H. pylori suşlarının antibiyotik duyarlılıkları metronidazol, klaritromisin, ampisilin, levofloksasin ve tetrasiklin için E-Test metodu ile yapılmıştır.E-Test ile yapılan antibiyotik duyarlılık testinde; amoksisiline %20, klaritromisine %38, levofloksasine %30, metranidazole %48 ve tetrasikline %10 oranlarında direnç saptanmıştır. E-Test; uygulamadaki kolaylığı, güvenilir ve kantitatif sonuçlar vermesi yönünden değerli bir test olarak kabul edilmiştir.
The aim of this study was to isolate Helicobacter pylori in biopsy specimens taken from patients who applied to Ondokuz Mayıs University School of Medicine Department of Gastroenterology Endoscopy Unit with gastroduodenal symptoms and perform its antibiotic susceptibility tests. For this purpose, antral biopsy specimens from patients with dyspeptic symptoms who admitted to Division of Gastroenterology Endoscopy Unit in our hospital between January 2012 and June 2012 have been investigated.Three biopsy samples were taken from gastric antrum of each patient and the existence of H. pylori was investigated by performing gram stain, urease test and culture. For culture, 7% defibrinated horse blood Brain Hearth Infusion agar supplemented with vancomycin, trimethoprim, cesulodin and amphotericin B (Helicobacter pylori Selective Supplement, SR0147E) was used. Inoculated culture media were incubated in anaerobic jars at 37 ° C for 3-10 days under microaerophilic conditions produced by GENbox kits. Antibiotic susceptibilities of isolated H. Pylori strains to metronidazole, clarithromycin, amoxicillin, levofloxacin and tetracyclin were investigated by E-test.In the antibiotic susceptibility test performed with test; 20% resistance to amoxicillin, 38% to clarithromycin, 30% to levofloxacin, 48% to metronidazole and 10% to tetracyclin was detected. E-test was considered a valuable test in terms of ease of application and giving reliable and quantitative results.
