Ovaryen Hiperstimülasyon Sendromu Gelişiminde Vegfr Geni Exon 7 Mutasyonlarının Etkisi

Abstract

Amaç: Yardımcı üreme teknikleri ile tedavi sırasında fazla yumurta oluşan ve OHSS (Ovaryen Hiperstimulasyon Sendromu) gelişen olgularda VEGF (Vasküler Endotelyal Büyüme Faktörü) reseptörü geninde meydana gelen genetik değişimleri inceleyerek uygulanan ilacın etkisinin kişiden kişiye farklılık göstermesine ve aynı dozun farklı kalitede yumurta oluşturmasına etkisinin incelenmesi amaçlandı. Materyal ve Metot: Ondokuz Mayıs Üniversitesi Tıp Fakültesi Tüp Bebek Merkezinde yardımcı üreme teknikleri ile tedavi gören ve istenilenden fazla sayıda yumurta elde edilen 27 hasta ve sağlıklı gebelik elde edilen 30 kadın kontrol, toplam 57 kişiden kan alındı. Kan örneklerinden kit yöntemi ile DNA elde edildikten sonra PCR (Polimeraz Zincir Reaksyonu) ve sonrasında DNA dizileme yöntemleri uygulanarak VEGFR Geni ekzon 7 bölgesi incelendi. Elde edilen sonuçlar SPSS ve Ki-kare analizi ile değerlendirildi. Bulgular: VEGFR geni ekzon 7 rs2305948 bölgesi açısından hasta ve kontrol grubunda genotip dağılımı bakımından istatistiksel olarak fark saptanmazken klinik verileri değerlendirildiğinde, CT genotipine sahip olan bireylerle yüksek sayıda yumurta oluşumu arasında istatistiksel olarak anlamlı derecede bir fark saptandı (p=0,006, χ²=7,560). Sonuç: Çalışmamızın sonucuna göre, yardımcı üreme tedavisi sürecinde VEGFR geni rs2305948 bölgesi bakımında CT genotipine sahip bireylerde çok daha yüksek sayıda yumurta oluştuğu ve OHSS gelişmesi açısından daha riskli grupta olduğu saptandı. Aim: We aimed to evaluate the pharmacogenetic effect of VEGFR gene mutations on the cases of have high number of ovum and develope OHSS during assisted reproductive technics. Material and Method: We evaluated 57 cases in this study. Twenty seven of them have high number of ovum during assisted reproductive technology in Ondokuz Mayis University Faculty of Medicine, IVF Center and 30 cases have healthy pregnancy. DNA isolated from all the cases. Polimerase Chain Reaction and following DNA sequencing methods performed for VEGFR gene exon 7 analysis. SPSS and Chi-square analysis performed for statistical analysis. Results: As a result of the study, there was no statistically significance between patients and control groups. However, when we analyse for clinical findings we found that frequency of CT genotype for VEGFR gene exon 7 variant was higher in the group of women have the high number ovum (p=0.006, χ²=7. 560). Conclusion: During assisted reproductive technics, the patients have CT genotype of VEGFR gene exon 7 may have high number of ovum ad under risk for OHSS.
