Publication: Hemşirelerin Diğerkamlık ve Merhamet Yorgunluk Düzeyleri ile Duygusal Emek Davranışları Arasındaki İlişki
Abstract
Amaç: Araştırma, hemşirelerin diğerkâmlık ve merhamet yorgunluk düzeyleri ile duygusal emek davranışları arasındaki ilişkiyi belirlemek amacıyla tanımlayıcı ve ilişki arayıcı bir çalışma olarak planlanmıştır. Materyal ve Metod: Araştırma 10 Ekim 2021-05 Mayıs 2023 tarihleri arasında Ondokuz Mayıs Üniversitesi Sağlık Uygulama ve Araştırma Merkezi Hastanesi'nde yürütüldü. Bu çalışmada nicel ve nitel araştırma yöntemleri birlikte kullanıldı. Çalışmanın nicel araştırma örneklemini 373, nitel araştırma örneklemini 20 hemşire oluşturdu. Nicel veriler Hemşireleri Tanıtıcı Bilgi Formu, Diğerkâmlık Ölçeği (DÖ), Merhamet Yorgunluğu-Kısa Ölçeği (MY-KÖ) ve Hemşirelerin Duygusal Emek Davranışı Ölçeği (HDEDÖ); nitel veriler yarı yapılandırılmış görüşme formu kullanılarak toplandı. Nicel verilerin analizinde IBM SPSS V23 ve IBM AMOS 24 programlarından, nitel verilerin analizinde MAXQDA programından yararlanıldı. Bulgular: DÖ toplam puan ortalaması 48,46±11,19, MY-KÖ toplam puan ortalaması 107,4±27,1 ve HDEDÖ toplam puan ortalaması 3,92±0,53 olarak belirlendi. DÖ, MY-KÖ ve HDEDÖ toplam ve alt boyut puan ortalamalarının hemşirelerin sosyo-demografik ve mesleki özelliklerine göre farklılık gösterdiği saptandı. DÖ toplam puanı ile HDEDÖ toplam puanı arasında istatistiksel olarak anlamlı pozitif yönlü orta şiddette bir ilişki olduğu belirlendi (r=0,607; p<0,001). HDEDÖ Yüzeysel Davranış, Derinlemesine Davranış ve Samimi Davranış alt boyutları ile DÖ arasındaki yol katsayısı istatistiksel olarak anlamlı bulundu (sırasıyla β=0,021; p<0,001; β=0,031; p<0,001; β=0,032; p<0,001). MY-KÖ toplam puanı ile DÖ puanı (r=-0,065; p=0,209); MY-KÖ toplam puanı ile HDEDÖ toplam puanı arasında (r=-0,031; p=0,547) istatistiksel olarak anlamlı bir ilişki olmadığı belirlendi. HDEDÖ Yüzeysel Davranış, Derinlemesine Davranış ve Samimi Davranış alt boyutları ile MY-KÖ Ölçeği arasındaki yol katsayısı istatistiksel olarak anlamlı bulunmadı (sırasıyla β=-0,001; p=0,513; β=0,001; p=0,447; β=0; p=0,879). Nitel verilerin analizi sonucunda; diğerkâmlığın anlamı, merhamet yorgunluğu, merhamet yorgunluğunun etkileri, merhamet yorgunluğuyla başa çıkma, duygusal emek davranışının anlamı, duygusal emek davranışının nedenleri, duygusal emek davranışının etkileri olmak üzere yedi tema 34 alt tema belirlendi. Sonuç: Bu araştırmada hemşirelerin diğerkâmlık düzeyleri ile duygusal emek davranışları arasında ilişki olduğu, hemşireleri diğerkâmlık düzeyleri arttıkça duygusal emek davranışlarının yükseldiği belirlendi. The study made combined use of quantitative and qualitative research approaches. 373 nurses made up the sample for the quantitative study, whereas 20 nurses made up the sample for the qualitative study. The Altruism Scale (AS), Compassion Fatigue-Short Scale (CF-SS), Nurses' Emotional Labor Behavior Scale (NELBS), and Nurses' Descriptive Information Form were used to gather quantitative data, while a semi-structured interview form was used to obtain qualitative data. Quantitative data were analyzed using the IBM SPSS V23 and IBM AMOS 24 programs, and qualitative data were analyzed using the MAXQDA tool. Findings: The mean total score from AS was 48,46±11,19, the mean total score from CF-SS was 107,4±27,1 and the mean total score from NELBS was 3,92±0,53. The mean total and subscale scores from AS, CF-SS and NELBS differed according to the socio-demographic and occupational characteristics of the nurses. There was a statistically significant and moderately positive relationship between the total score from AS and the total score from NELBS (r=0.607; p<0.001). The path coefficient between the Superficial Behavior, In-depth Behavior and Sincere Behavior sub-dimensions of NELBS and AS was statistically significant (β=0,021; p<0,001; β=0,031; p<0,001; β=0,032; p<0,001, respectively). There was no statistically significant relationship between the total score from CF-SS and AS (r=- 0,065; p=0,209) nor between the total score from CF-SS and NELBS (r=-0,031; p=0,547).
