Abstract
ÖZET AMAÇ: Albuminüri varlığının, ağır hipertansiyon ve son organ hasarının bulgularıyla ilişkili olduğu, hipertansif hastalardaki vasküler endotel dİsfonksiyonunu ve artmış kardiyovasküler riski erken dönemde gösterdiği bilinmektedir. Biz bu çalışmamızda, hastanemize hipertansiyonla başvuran hastalardaki endotel disfonksiyonu ile mikroalbunıinüri arasındaki ilişkiyi göstermeyi amaçladık. METODl Çalışmaya hipertansiyonu ve sol ventrikiil hipertrofîsi olan 60 hasta alındı. Hastalar mikroalbummürinin varhğına göre; Mikroalbuminürisi olan 30 hasta ve mikroalbuminürisi olmayan 30 hasta olarak 2 gruba ayrıldı. Endotel fonksiyonlarının belirlenmesi amacıyla tüm hastalara vasküler ultrasonografî uygulandı. BULGULAR: Çalışmaya alınan her iki hasta grubunun özellikleri birbirine benzerdi. Endotel disfonksiyonu olan grupta LVMI 166,8±44,6, IVS 1,5±2, PW 1,32±1,3 ve LVEDÇ 49±12, endotel disfonksiyonu olmayan grupta ise LVMI 157±34,2, VS 1,38±1,5, PW 1,28±1,3 ve LVEDÇ 46±11 olarak saptanırken parametreler arasında istatistiksel bir fark gözlenmedi. Mikroalbuminürik grupta 29 (%96,7) hastada endotel disfonksiyonu saptandı, sadece 1(%3,3) hastada endotel fonksiyonları normaldi, normoalbüminürik hastalardan ise 6 (%20) hastada endotel disfonksiyon saptanırken, 24 (%80) hastada endotel fonksiyonları normaldi. Endotel disfonksiyonu açısından mikroalbuminürik ve normoalbüminürik hasta grupları arasında istatistiksel olarak anlamlı bir fark bulundu (p< 0001). SONUÇ: Çalışmamızda mikroalbuminürik grupta belirgin endotel disfonksiyonu saptanırken, normoalbüminürik grupta endotel fonksiyonları normal olarak bulunmuştur. Albuminüri değiştirilebilir bir risk faktörü olduğu için, kardiyovasküler hastalıkların patogenezinde rol oynayan lipid ve kan basmcı gibi değişkenlerde yapıldığı gibi albuminüri içinde tarama yapılması önerilebilir. ABSTRACT OBJECTIVE: Albuminuria has been believed to have relations between uncontrolled hypertension and end stage organ damage findings, may reflect vascular endothelial dysfunction in hypertension and determine increased risk of cardiovascular disorders in the earlier periods. We aimed to investigate a degree of relationship between endothelial dysfunction and microalbuminuria in hypertensive patients. METHOD: 60 consecutive patients with hypertension and left ventricular hypertrophy were enrolled to the study. The patients were divided according to presenting mic-roalburninuira into 2 group: 30 patients with microalbuminuria and 30 patients without microalbuminuria. All patients were undergone vascular ultrasonography to detect en-dothelial dysfunction. RESULTS: Patient characteristics were similar between groups. In the group with endothelial dysfunction parameters were as followed: LVMI 166,8±44,6,IVS 1,5±2,PW 1,32±1,3 ve LVEDD 49±12, in the patients without endothelial dysfunction were LVMI 157±34,2,IVS 1,38±1,5,PW 1,28±1,3 ve LVEDD 46±11, respectively. There were no any statistical differencies between parameters. Endothelial dysfunction was found in 29(%96,7) patients with microalbuminuria, only 1(%3,3) patient in this group had normal endothelial function. Otherwise, endothelial dysfunction was found in 6 (%20) normoalbuminuric patients, but 24 (%80) reminder patients had normal endothelial function. According to endothelial dysfunction there were significant statistical differencies between normo- and microalbuminuric patients (p< 0001). CONCLUSION: We found marked endothelial dysfunction in the rnicroalbuminuric group, but in the normoalbuminuric group endothelial mention were normal in the most of patients. Albuminuria is a modifiable risk factor and together with lipids and blood pressure which have a role in pathogenesis of cardiovascular disorders may recommend as a screening target.
