Abstract
1.GİRİŞ ve AMAÇDalağın cerrahi hastalıklarının çoğunluğunu medikal tedaviye dirençli ve şiddetli seyreden hematolojik hastalıklar oluşturur. Bu çalışmada, kliniğimizde hematolojik nedenlerle tanı ve tedavi amaçlı splenektomi yapılan hastalar ve sonuçlarını sunmayı amaçladık.2.GEREÇ ve YÖNTEMBu çalışmada Eylül 2005 ile Mayıs 2011 tarihleri arasında Samsun Ondokuz Mayıs Üniversitesi Tıp Fakültesinde splenektomi yapılan 184 hasta retrospektif olarak incelendi. Çalışmaya, bu hastalardan Hematoloji Bilim Dalına başvuru yapan ve hematolojik hastalığı nedeni ile splenektomi yapılan 54 hasta dahil edildi.3.BULGULARYaş ortalaması 43,93±16,9 olup, %63 hasta kadın %37 hasta ise erkek idi. Splenektomi yapılan hematolojik hastalıklar; İmmüm trombositopenik purpura, Herediter sferositoz, Otoimmün hemolitik anemi, Hairy cell lösemi, Hodgkin lenfoma, Kronik lenfositik lösemi idi. En sık splenektomi yapılan hematolojik hastalık İTP (%70,4) idi. İTP hastalarında K:E oranı 2 olarak tespit edildi. Hairy cell lösemi nedeniyle splenektomi yapılan 4 hastanın 3'ü erkek idi. Hastaların %70,4'ünde dalak boyutu normal saptandı. İTP nedeniyle splenektomi yapılan hastaların %92.1'inde dalak boyutu normal idi. Ameliyat sırasında %13 hastada aksesuar dalak tespit edildi. Hastaların hastaneye en sık başvuru semptomu cilt lezyonları (%35,19) ve kanama (%27,78) idi. Hastaların 3'ünde aile öyküsü mevcutdu. 3 hasta da HS hastası idi. HS'lu 2 hastaya safra taşı nedeniyle kolesistektomi de yapıldı. Gebe olan bir İTP hastasına splenektomi ile sezaryan eş zamanlı uygulandı. Hiç bir hastada operasyon sırasında komplikasyon ve ölüm görülmedi.4.SONUÇSonuç olarak; hematolojik hastalıklarda splenektomi endikasyonlarının günümüzdeki tanı yöntemleri ve teknolojinin gelişmeleri ışığı altında her geçen gün daha fazla önem kazandığı düşüncesindeyiz.Anahtar kelimeler: Hematolojik hastalık, Splenektomi, Analiz
1.INTRODUCTIONThe majority of surgical diseases of the spleen is consist of hematological diseases which are severe and resistant to medical treatment. İn this study, we aimed to present patients who had a diagnostic and therapeutic splenectomy history because of hematological reasons and their results in our clinic.2.MATERIALS AND METHODSİn this study, between September 2005 and May 2011, 184 patients with splenectomy were retrospectively reviewed who are follow up in Ondokuz Mayıs University Samsun Faculty of Medicine. We included in the study 54 patients of 184 patients who applied to hematology field of study and with splenectomy due to hematological disease.3.RESULTSThe mean age was 43,93±16,9 , 37% of patients were men, 63% of patients were women. Hematologic diseases with splenectomy were İTP, HS, OHA, HCL, HL, KLL. The most common hematologic disease in splenectomized was İTP (70.4%). İn İTP patients, W: M ratio was determined as 2. 3 of 4 patients with hairy cell leukemia, splenectomized were male. 70,4% of the patients have a normal size spleen. 92,1% of patients who splenectomized due to İTP have a normal size spleen. Accessory spleen was found in 13% of patients during surgery. The most common presenting symptom of patients, were skin lesions (35,19%) and bleeding (27,78%), respectively. Family history was available in 3 patients. 3 patients were suffering from HS. Cholecystectomy was performed in 2 patients of 3 patients suffering from HS due to gallstone. 1 of splenectomized İTP patients who was pregnant performed splenectomy and cesarean section simultaneously. There were no complications and death in a patient during surgery.4.DISCUSSION AND CONCLUSİONAs a result, indications for splenectomized in hematological diseases in the light of present-day developments in diagnostic methods and technology became more important with each passing day, we think.Key words: Hematologic disease, Splenectomy, Analysis
