Regmatojen Retina Dekolmanında Cerrahi Sonuçlarımız

Abstract

Amaç: Kliniğimizde regmatojen retina dekolmanı (RRD) tanısı ile dekolman cerrahisi uygulanan hastalarda klinik özelliklerin ve cerrahi sonuçların tanımlanması.Gereç ve Yöntem: Ekim 2005-Ekim 2011 tarihleri arasında Ondokuz Mayıs Üniversitesi Tıp Fakültesi Göz Hastalıkları ABD'de RRD tanısı ile retina dekolmanı (RD) cerrahisi geçiren 225 hastanın 230 gözünü içeren kayıtlar retrospektif olarak incelendi. Hastalar geçirmiş oldukları cerrahiye göre 4 gruba ayrıldı: skleral çökertme (SÇ), pnömotik retinopeksi (PR), pars plana vitrektomi (PPV) ve serklajla kombine vitrektomi (kombine). Ortanca takip süresi 4 ay (1-56 ay) idi. Her bir grupta ilk muayenedeki klinik özellikler, primer cerrahi başarı (PCB), 3.ay, 6.ay, 1.yıl ve son takipteki anatomik başarı (AB) ve fonksiyonel başarı (FB), nüks RD gelişimi oranları incelendi. PCB, primer cerrahiyi takiben 4 hafta içerisinde retinanın yatışık olması ve tekrar cerrahi gerekliliğin görülmemesi olarak tanımlanırken; AB, fundus muayenesinde retinanın yatışık olması, FB ise en iyi düzeltilmiş görme keskinliği (EİDGK) ? 0,1 olarak tanımlandı.Bulgular: SÇ, PR, PPV ve kombine gruptaki hasta sayıları sırasıyla 145 (%63), 15 (%6,5), 53 (%23,1) ve 17 (%7,4) idi. Primer cerrahi sonrası SÇ grubunda 32 gözde (%21,9), PR grubunda 11 gözde (%73,3), PPV grubunda 6 gözde (%11,5) retinada yatışıklık sağlanamadı. Proliferatif vitreoretinopati (PVR)'li gözlerde PCB oranı %68,3, PVR'siz gözlerde %82 idi (p=0,48). 6.aydaki AB ve FB oranları sırasıyla SÇ grubunda % 93,4/67,2, PR grubunda % 100/80, PPV grubunda % 81/42,9 ve kombine grupta % 80/70 idi (p=0,1). Fakik gözlerin %94,1'inde (64 göz), nonfakik gözlerin ise %82,4'ünde (28 göz) AB sağlandı (p=0,06). Makulanın dekole olduğu gözlerde FB %60,3, dekole olmadığı gözlerde ise %71,4 idi (p<0,05). Takip süresince 30 hastada (%13) nüks RD gelişimi gözlendi. Nüks gelişen hastalarda PVR olma olasılığı gelişmeyenlere göre 5,6 (2,2?14,5) kat daha fazla bulundu. Cerrahi sonrası en sık görülen komplikasyon %29 oran ile GİB artışıydı.Sonuç: Primer cerrahi başarı SÇ ve PPV ile karşılaştırıldığında istatiksel anlamlı şekilde PR'de düşükken; kombine cerrahide yüksektir. Primer yatışıklıkta PPV'nin SÇ'ye üstünlüğü yoktur. Final anatomik ve fonksiyonel başarı açısından cerrahiler arasında anlamlı fark yoktur.Anahtar kelimeler: Regmatojen retina dekolmanı, skleral çökertme, vitrektomi, pnömotik retinopeksi

Objective: To describe the clinical characteristics and the results of surgery in the patients who underwent detachment surgery with the diagnosis of rhegmatogenous retinal detachment (RRD) in our clinic.Material and method: The charts of two hundreds and thirty eyes of two hundreds and twenty five patients who underwent retinal detachment (RD) surgery with the diagnosis of RRD in the Department of Ophthalmology of Ondokuz Mayıs University Faculty of Medicine between October 2005 and October 2011 were revieved retrospectively. Patients were divided into four groups according to the surgery they underwent: scleral buckle (SB), pneumatic retinopexy (PR), pars plana vitrectomy (PPV) and vitrectomy combined with an encircling band (combined). The median follow-up time was 4 months (1-56 months). The clinical characteristics in the first examination, primer success of surgery (PSS), the anatomical success(AS) and functional success (FS) at the third month, sixth month, first year and final visit, the rates of redetachment were analysed in each group. When PSS is defined as retinal reattachment and no necessity of a new surgery within four weeks after primary surgery, AS is defined as being retinal attachment at fundus examination and FS is defined as being best corrected visual acuity (BCVA) ? 0,1.Results: The number of patients in SB, PR, PPV and combined group was 146 (63,5%), 15 (6,5%), 52 (22,6%) and 17 (7,4%) respectively. RD was persisted in 32 eyes (21,9%) in SB group, 11 eyes (73,3%) in PR group and 6 eyes (11,5%) in PPV group after the primer surgery. While the primer success of surgery in the eyes with proliferative vitreoretinopathy (PVR) was 68,3%, it was 82% in the eyes without PVR (p=0,48). AS and FS rates at the sixth month were 93,4/67,2 % in SB group, 100/80 % in PR group, 81/42,9 % in PPV group and 80/70 % in combined group respectively. AS has gained in 94,1% (64 eyes) of phakic eyes and 82,4% (28 eyes) of nonphakic eyes (p=0,06). The FS rate was 60,3% (44 eyes) in the eyes with macular detachment and 71,4% (20 eyes) in the eyes without (p<0,05). Retinal redetachment was seen in 30 eyes (13%) during the follow-up period. The likelihood ratio of PVR in redetached eyes was 5,6 (2,2-14,5) times more than in attached eyes. The increase in İOP was the common complication seen with the rate of 29% after the surgery.Conclusion: The success of primer surgery is statistically significant low in the PR group and high in the combined group when compared to SB and PPV. PPV has no significant superiority over SB in primer reaattachment. There is no statistically significant difference among the groups in the means of final anatomic and functional success.Key words: Rhegmatogenous retinal detachment, sckleral buckle, vitrectomy, pneumatic retinopexy
