Spontaneous bilateral intracerebral hemorrhages are extremely rare. To further characterize this rare event, we report a 60-year-old man with chronic hypertension, who presented with bilateral subthalamic hemorrhages that consequently resulted in monoplegia in the right upper extremity and loss of consciousness. The literature is reviewed regarding this condition.en_US
