Hodgkin ve Diffüz Büyük B Hücreli Lenfoma Hastalarında Tanı Anındaki Vücut Kitle İndeksinin Sağkalım Üzerindeki Etkisinin Retrospektif Olarak İncelenmesi

Abstract

HODGKİN VE DİFFÜZ BÜYÜK B HÜCRELİ LENFOMA HASTALARINDA TANI ANINDAKİ VÜCUT KİTLE İNDEKSİNİN SAĞKALIM ÜZERİNDEKİ ETKİSİNİN RETROSPEKTİF OLARAK İNCELENMESİ Amaç: Gelişmiş ülkelerde kanserden ölüm, ölüm sebepleri arasında kalp hastalıklarından sonra ikinci sırada yer almaktadır. Lenfomalar B, T ve doğal öldürücü hücrelerden köken alan hematopoetik tümörlerdir. Lenfoma ergenlik çağının en sık malignitesidir. Lenfoma insidansı; yaş, coğrafi bölge, enfeksiyöz etkenlere maruziyet, ırksal faktörler ile değişmektedir. Dünyada obezite sıklığı her geçen yıl artmaktadır. Amacımız lenfoma hastalarında tanı anındaki vücut kitle indeksinin (VKİ) genel sağkalıma (OS) ve progresyonsuz sağkalıma (PFS) etkilerini araştırmaktır. Yöntem: Çalışma grubunu, Ocak 2005 ? Ocak 2013 tarihleri arasında, 19 Mayıs Üniversitesi İç Hastalıkları Hematoloji kliniğinde Hodgkin (HL) ve Diffüz Büyük B hücreli Lenfoma (DLBCL) tanısı alan ve takibi yapılan toplam 378 olgu oluşturdu. Çalışmamızda; HL ve DLBCL hastalarında hastalık evresinin, performans durumunun, B semptom varlığının, IPI ve IPS skorunun genel sağkalıma ve progresyonsuz sağkalıma; tanı anındaki vücut kitle indeksinin genel sağkalıma ve progresyonsuz sağkalıma etkileri araştırıldı. Bulgular: Hastaların 120?si HL, 258?i DLBCL tanılı idi. HL hastalarının ortanca yaşı 42 (18-84), DLBCL hastalarının ise ortalama yaşı 60.17±14.92 (18-87) idi. HL hastalarının VKİ ortalaması 26.22±5.77 (13.91-42.98) kg/ m2, DLBCL hastalarının ise VKİ ortalaması 26.18±5.45 (14.36-55.78) kg/m2 idi. Tanı anında AnnArbor evreleme sistemine göre HL hastalarından 23?ü (%19.2) Evre I, 68?i (%56.7) Evre II, 21?i (%17.5) Evre III ve 8?i (%6.7 ) Evre IV olarak; DLBCL hastalarından ise 53?ü (%20.5) Evre I, 125?i (%48.4) Evre II, 50?si hasta (%19.4) Evre III ve 30?u (%11.6 ) Evre IV olarak belirlendi. HL hastalarında VKİ ile OS, VKİ ile PFS arasında; DLBCL hastalarında VKİ ile OS, VKİ ile PFS arasında istatiksel açıdan anlamlı fark bulunmadı. Sonuç: Literatürde VKİ ile lenfoma sağkalımı arasındaki ilişkiyi inceleyen birbiri ile zıt sonuçları olan çalışmaların olması nedeni ile bu konu ile ilgili daha çok çalışmanın yapılması gerektiğini düşünüyoruz. RETROSPECTIVE STUDY OF THE EFFECT OF THE BODY MASS INDEX AT DIAGNOSIS ON SURVIVAL IN HODGKIN AND DIFFUSE LARGE B CELL LYMPHOMA PATIENTS Aim: In developed countries, cancer mortality, causes of death among heart diseases ranks second after. Lymphomas are hematopoietic tumors derived from B, T and Natural Killer cells. Lymphoma is the most common of puberty malignancy. The incidence of lymphoma varies with age, geographic region, infectious exposures and racial factors. The prevalence of obesity in the world is increasing with each passing year. Our goal is to investigate the effect of body mass index in patients with lymphoma at diagnosis, on overall survival (OS) and progression-free survival (PFS). Material and Method: In working group, there were a total of 378 patients with a diagnosis and follow_up of Hodgkin Lymphoma (HL) and Diffuse Large B Cell Lymphoma (DLBCL) between January 2005 - January 2013 in 19 Mayis University Clinic of Internal Medicine Hematology. In our study we investigated the effect of disease stage, performance status, the presence of B symptoms, IPI and IPS score, body mass index (BMI) in patients at diagnosis, on OS and PFS. Results: 120 patients was diagnosed with HL and 258 diagnosed with DLBCL. The median age of HL patients was 42 (18-84), the mean age of DLBCL patients was 60.17 ± 14.92 (18-87), respectively. The average BMI of HL patients with was 26.22 ± 5.77 (13.91-42.98) kg /m2, in patients with DLBCL it was 26.18 ± 5.45 (14.36-55.78) kg/m2. By the time of diagnosis, according to staging system of AnnArbor 23 (19.2%) of HL patients in stage I, 68 (56.7%) in stage II, 21 (17.5%) in stage III, and 8 (6.7%) in stage IV; 53 (20.5%) of DLBCL patients in stage I, 125 (48.4%) in stage II, 50 (19.4%) in stage III, and 30 (11.6%) were in stage IV. No statistically significant difference was found between BMI and OS, BMI and PFS in both HL and DLBCL patients by the time of diagnosis. Conclusion: We think that there must be more study needs to be done because of the contrasting results of studies dealing with this issue on the relationship between BMI at diagnosis and survival of lymphoma in the literature.
