Publication: Diz Osteoartriti Belirtileri, Ağrısı, Sertliği, Fonksiyonel Durumu ve Yaşam Kalitesi İle Vücut Kitle İndeksi ve Bel Çevresinin İlişkisi: Kesitsel Çalışma
Abstract
Amaç: Bu çalışmada diz osteoartiriti belirtileri, ağrısı, sertliği, fonksiyonel durumu ve yaşam kalitesi ile vücut kitle indeksi ve bel çevresinin ilişkiyi değerlendirmesi amaçlanmıştır. Hastalar ve Yöntem: Çalışma Nisan -Ekim 2023 tarihleri arasında, Ondokuz Mayıs Üniversitesi (OMÜ) Tıp Fakültesi aile hekimliği anabilim dalına bağlı eğitim aile sağlığı merkezlerine başvuran daha önce Ortopedi ve Travmatoloji veya Fizik Tedavi ve Rehabilitasyon bölümü hekimleri tarafından primer diz osteoartrit tanısı konulan ve e-nabız sisteminde her iki diz radyolojik görüntülemesi bulunan hastalar tarafımızca Amerikan Romatoloji Konseyi(ACR) kriterlerine göre yeniden değerlendirilerek tanısı doğrulanan hastalar arasından yapılacaktır. Anket formu 4 bölümden oluşmaktadır. Anket bölümlerinde katılımcı özellikleri, dizdeki osteoartritin radyografik şiddeti Kellgren ve Lawrence skalası, dizdeki ağrı derecesinin değerlendirilmesi görsel analog skalası (VAS) ve diz osteoartiriti belirtileri, ağrısı, sertliği, fonksiyonel durumu ve yaşam kalitesini değerlendirmek için Diz Yaralanması ve Osteoartrit Sonuç Skoru (KOOS) sorgulanacaktır. Bulgular: Çalışmaya toplamda 280 kişi katılmıştır. Katılımcıların %76,1'i kadın ve yaş ortalamaları 55,16±11,06 idi. Şikayetin süresi ortalama 4,6 yıl, vücut kitle indeksi ortalama 31,4 ve bel çevresi ölçümü ortalama 92,71 cm idi. Katılımcıların %85,7 si obez, %54,6'sı abdominal obeziteydi. Obezitesi ve/veya abdominal obezitesi olan hastalar KOOS ve alt boyutları incelendiğinde dizde sertlik, ağrı, günlük yaşam aktivitesi, spor ve boş zaman değerlendirme aktiviteleri ve yaşam kalitesi açısından istatistiksel olarak anlamlı şekilde olumsuz etkilendiği görüldü. Obezitesi ve/veya abdominal obezitesi olan hastaların görsel ağrı skoru ,obezitesi ve/veya abdominal obezitesi olmayan hastalara göre istatistiksel olarak anlamlı şekilde daha yüksekti. Sonuçlar: Bu çalışma ile erişkin popülasyonunda obezitesi ve/veya abdominal obezitesi olan kişilerin dizde sertlik, ağrı durumunun anlamlı şekilde arttığını ve günlük yaşam aktivitesi, spor ve boş zaman değerlendirme aktiviteleri ve yaşam kalitesi açısından olumsuz etkilendikleri sonucuna ulaşılmıştır. Objective: The aim of this study was to evaluate the association of body mass index and waist circumference with symptoms, pain, stiffness, functional status and quality of life in osteoarthritis of the knee. Patients and Methods: The study will be conducted between April and October 2023 among the patients who applied to the training family health centers affiliated to the department of family medicine of Ondokuz Mayıs University (OMU) Faculty of Medicine, who were previously diagnosed with primary knee osteoarthritis by Orthopedics and Traumatology or Physical Therapy and Rehabilitation department physicians and who had radiological imaging of both knees in the e-pulse system and whose diagnosis was confirmed by re-evaluation according to the American Council of Rheumatology (ACR) criteria. The questionnaire consists of 4 sections. In the questionnaire sections, participant characteristics, radiographic severity of osteoarthritis in the knee using the Kellgren and Lawrence scale, visual analog scale (VAS) to assess the degree of pain in the knee, and Knee Injury and Osteoarthritis Outcome Score (KOOS) to assess knee osteoarthritis symptoms, pain, stiffness, functional status and quality of life will be questioned. Results: A total of 280 people participated in the study. 76.1% of the participants were female and the mean age was 55.16±11.06 years. The mean duration of the complaint was 4.6 years, the mean body mass index was 31.4 and the mean waist circumference was 92.71 cm. 85.7% of the participants were obese and 54.6% had abdominal obesity. When KOOS and its sub-dimensions were analyzed, it was seen that patients with obesity and/or abdominal obesity were statistically significantly negatively affected in terms of knee stiffness, pain, activities of daily living, sports and leisure time activities and quality of life. Visual pain score of patients with obesity and/or abdominal obesity was statistically significantly higher than patients without obesity and/or abdominal obesity. Conclusion: In this study, it was concluded that people with obesity and/or abdominal obesity in the adult population had significantly increased knee stiffness and pain and were negatively affected in terms of daily living activities, sports and leisure time activities and quality of life.
