Abstract
ÖZET Kemik lezyonlarının bazıları, klinik ve radyolojik inceleme ile kolay tanınabilmektedir. Ancak, kemik tümörü olan bir çok olguda, cerrahi evrelendirme yapmak ve farklı tedavi gerektirebilecek tümörün tipini belirlemek için histopatolojik tanı gereklidir. Bu çalışmanın amacı, kemik tümöründen şüphelenilen lezyonların tanısında, BT eşliğinde yapılan perkütan iğne biyopsisinin doğruluğunu ve avantajlarını değerlendirmektir. Çalışmamızda, 1995 ile 2001 tarihleri arasında, kemik lezyonları olan ve biyopsi amacı ile Radyodiagnostik Anabilim Dalına gönderilen 67 hastaya BT eşliğinde 71 iğne biyopsisi yapılarak alınan histopatolojik sonuçlar tartışılmıştır. Altmışyedi hastaya yapılan toplam 71 iğne biyopsisinin 53'üne (%74.65) doğru olarak malign ve nonmalign tanı konuldu. Son tanısı malign olan 3 1 iğne biyopsisinden 29'una (%93.55), son tanısı nonmalign olan 40 iğne biyopsisinden 24'üne (%60) doğru tanı konuldu. Geri kalan 18 biyopsi (%25.35) ile yapılan doku örneklemesinin histopatolojik sonucu yetersiz olarak değerlendirildiğinden tanı konulamadı. Kemik tümöründen şüphelenilen olgularda, BT eşliğinde yapılan perkütanöz iğne biyopsileri; radyolog, patolog ve ortopedistten oluşan bir ekiple beraber çalışıldığında, hızlı, güvenilir, ucuz, travmatize edici etkisi ve komplikasyon oranı düşük, doğruluk derecesi yüksek ve hastaların yatışına gerek kalmadan uygulanan bir tanı yöntemidir. ABSTRACT A number of bone lesions can be readily diagnosed by clinical and radiological examination. However, histopathologic diagnosis is necessary to determine the theurapeutic strategy and surgical staging. The aim of this study is to evaluate the accuracy and advantages of CT guided percutaneous needle biopsy in the diagnosis of lesions having suspicion of bone tumor. In this study, CT guided 71 needle biopsies were performed in 67 patients with bone lesions referred to Radiodiagnostic Department for biopsy between 1995 and 2001, and histopathologic results were discussed. Of the 71 needle biopsy performed in 67 patients, 53 (74.65%) was diagnosed accurately as either malign or nonmalign. Of the 31 lesions with a final malign diagnosis, 29 (93.55%) had accurate diagnosis, whereas of the 40 lesions with a final benign diagnosis, 24 (60%) had accurate diagnosis. In the remaining 18 biopsy a diagnosis could not be obtained since the histopathologic result of tissue sampling was inadequate. CT guided percutaneous needle biopsy, performed by a team consisted of radiologist, pathologist and orthopedist in cases with suspicious bone tumor, is a fast, safe, cheap and less traumatizing diagnostic method with low complication rate, high accuracy and no need for hospitilization.
