Abstract
Özet Prostat kanserlilerde VDR gen polimorfizmleri Taq I, Bsm I ve Apa I ile kemik metastazı olanlar ile olmayanlar arasındaki fark ve PSA ve Gleason skorları ile ilişkisi incelendi. Kemik sintigrafisinde metastatik lezyon görülmeyen 57 (grup 1), görülen 50 (grup 2) prostat kanserli ve 61 BPH olgusunda (grup 3) VDR geninin ekson 9 daki Taq I, Apa I ve intron 8 deki Bsm I RFLP tanımlandı. Alellerin varlığına veya yokluğuna göre TT+Tt vs tt, Aa+aa vs AA ve Bb+bb vs BB lerin odds ratio hesaplandı. VDR Taq I polimorfizminde tt genotip sıklığı grup 1,2 ve 3 de sırası ile %17.5, %8 ve %13.1 oranlarında görüldü. Ancak aradaki fark istatistiksel olarak anlamlı değildi(p=0.424). Gleason skoru ve PSA seviyesine göre değerlendirildiğinde Gleason skoru >7 dekilerde OR = 2.80 (%95 Cl 0.66 - 12.63; p=0.267) iken serum PSA seviyesi >20 olanlarda BB genotipi görülme sıklığı 3.5 kat fazlaydı ve aralarındaki fark istatistiksel olarak anlamlıydı (p= 0.014). Ayrıca Gleason skoru >7 bulunanlarda kemik metastazı olmayanların tt genotipi kemik metastazı olanlardan 9 kat yüksekti (p=0.081). Aynı grupta kemik metastazı olanlarda BB genotipi yaklaşık 3 kat fazlaydı (p=0.077). tt genotipinin iki değişkenli analizinde Gleason skoru > 7 olanlarda serum düzeyi PSA<20 ng/ml değerindekilerde daha yüksek bulunanlara göre 9 kat daha fazlaydı (p=0.083). Gleason skoru ^7 dakilerin PSA seviyelerine göre tt genotipi incelendiğinde aralarında istatistiksel olarak anlamlı bir fark bulunamadı. Gleason skoru £7 lerin VDR Bsm I RFLP de PSA seviyesine göre odds ratio PSA >20 grupta yaklaşık 9 kat fazlaydı (p=0.04). Sonuç olarak prostat kanserinde VDR gen polimorfizmi kemik metastazı olan ve olmayanlar arasında önemli farklılık göstermemektedir. BB genotipi görülme sıklığı Gleason skoru >7 ve serum PSA seviyesi >20 ng/ml olanlarda istatiksel olarak anlamlı farklılık göstermektedir. Bu bulgular prostat kanserinin genetik yönünün daha detaylı araştırılması gerektiğini göstermektedir. VII
ABSTRACT In prostate cancer patients VDR gene polymorphism and Taq I, Bsm I and Apa I were studied and their levels relations with bone metastasis, PSA levels and Gleason scores were investigated. In 57 prostate cancer cases without metastasis according nucleer bone scans ( group-1), in 50 cases with metastasic lesions in the bone scans (group-2 ), and in 61 BPH cases ( group 3) Taq I, Apa I RFLP in exon 9 and Bsm I RFLP in intron 8 of VDR gene were identified. According to the presence and absence of the alleles, the odds ratios of TT+ Tt vs. tt, Aa +aa vs. AA and Bb + bb vs. BB were calculated. In VDR Taq I polymorohism, the frequency of tt genotype in groups 1,2 and 3 were 17.5 %, 8%, and 13.1% respectively. But the difference was not statistically significant (p=0.424). In whom Gleason scores were > 7, OR was 2.80(95% CI 0.66 - 12.63; p=0.267), and in cases whose PSA levels were > 20 BB genotype was found to be 3.5 folds higher and the difference was statistically significant (p=0.014). Also,in cases with > 7 scores, tt genotype was 9 folds higher in cases whithout bone metastasis compared to the cases with metastasis.(p=0.081) In the same group, BB genotype was found 3 folds higher in cases whit bone metastasis. (p=0. 077) When tt genotype was evaluated with 2 variables, in cases with Gleason score > 7, the number serum the patient was 9 folds PSA s 20 ng/ml of the cases with higher PSA. In cases with £ 7 Gleason scores, there was no statistically significant difference when tt genotype was evaluated according to the PSA levels. In cases with Gleason score < 7 and had VDR Bsm I RFLP, the odds ratio, according to the PSA levels, was about 9 folds higher in PSA > 20 group (P=0.004). In conclusion, in prostate cancer VDR gene polymorphism does not exhibit significant difference between the cases with and without bone metastasis BB genotype freguency exhibits statistically significant difference in cases whit Gleason score > 7 and PSA levels > 20 these findings suggest genetic aspect of the prostate cancer should he the subject of further detailed investigations.
