Publication: Hemodiyaliz Tedavisi Alan Hastaların Belirsizlik Durumlarının Evde Bakım Gereksinimleri Üzerine Etkisi
Abstract
HEMODİYALİZ TEDAVİSİ ALAN HASTALARIN BELİRSİZLİK DURUMLARININ EVDE BAKIM GEREKSİNİMLERİ ÜZERİNE ETKİSİ Salim ÖZBEK Ondokuz Mayıs Üniversitesi Lisansüstü Eğitim Enstitüsü Evde Hasta Bakım Hemşireliği Ana Bilim Dalı Yüksek Lisans, Haziran/2024 Danışman: Dr. Öğr. Üyesi Figen ÇAVUŞOĞLU Amaç: Bu araştırmada hemodiyaliz tedavisi alan hastaların hastalıkta belirsizlik durumlarının evde bakım gereksinimleri üzerine etkisinin incelenmesi amaçlanmıştır. Materyal ve Metot: Tanımlayıcı ve kesitsel türde planlanan bu araştırma Sinop ilinde hemodiyaliz tedavisi alan araştırmaya katılmayı kabul eden 158 hasta ile Ocak-Haziran 2023 tarihleri arasında gerçekleştirilmiştir. Araştırmanın verileri 'Tanımlayıcı Bilgi Formu', 'Hemodiyaliz Tedavisi Alan Hastaların Evde Bakımı Gereksinimlerinin Belirlenmesine Yönelik Anket' ve 'Mishel Hastalıkta Belirsizliği Ölçeği-Toplum Formu' kullanılarak yüzyüze toplanmıştır. Verilerin analizinde tanımlayıcı istatistiklerle birlikte, bağımsız örneklem t testi, varyans analizi, pearson korelasyon analizi ve logistik regresyon analizi kullanılmıştır. Araştırma kapsamında etik kurul, kurum izni ve hastalardan onam alınmıştır. Bulgular: Araştırmaya katılanların %74.1'i hemodiyalize bağlı iş hayatında, %51.3'ü aile içi ilişkiler ve rolleri yerine getirmede sorunlar yaşadığı ve %88'nin çalışmadığı bulunmuştur. Katılımcıların evde bakımla ilgili durumları incelendiğinde ise %71.5'inin evde bakımı ile ilgilenen birisi olduğu ve %78.5'inin evde bakımla ilgili kendi yada bakım vereninin herhangi bir eğitim almadığı saptanmıştır. Katılımcıların Hastalıkta Belirsizlik Ölçeği toplam puanı 58.96±9.60 olarak tespit edilmiştir. Hemodiyaliz tedavisi gören hastaların %58.9'u alışveriş yapma konusunda, %52.5'i herhangi bir yere seyahat etme, %65.8'i de yorgunluk ve halsizlik yaşanıldığında yardıma gereksinimi olduğunu belirtmiştir. Evde bakım gereksinimleri arasında kişisel temizlik, sıvı kısıtlaması, kıyafet değiştirme ve en içinde ya da dışarıda hareket etme konularında gereksinim duymanın hastalık belirsizliği üzerinde en fazla oranda anlamlı yordayıcı olduğu tespit edilmiştir (p<0.05). Hastalığı ile bilgili yeterli bilgiye sahip olanların daha az düzeyde belirsizlik yaşadıkları bulunmuştur (p<0.05). Sonuç: Çalışma sonucunda; hemodiyaliz tedavisi alan hastalarda orta düzeyde hastalıkta belirsizlik yaşadıkları, hastalığı ile ilgili ve kişisel ihtiyaçlarla ilgili belirlenen gereksinimlerin hastalıkta belirsizlik üzerinde yordayıcı etkisi olduğu saptanmıştır. Diyaliz tedavisi gören hastalara süreç ile ilgili daha fazla düzeyde bilgi verilmesi ve evde bakım gereksinimleri doğrultusunda desteklenmesi önerilmektedir. THE EFFECT OF UNCERTAİNTY İN SİTUATİONS OF PATİENTS RECEİVİNG HEMODİALYSİS TREATMENT ON HOME CARE NEEDS Salim ÖZBEK Ondokuz Mayıs University Department Of Home Care Nursing Master, June/2024 Supervisor: Assoc. Prof. Dr. Figen ÇAVUŞOĞLU Purpose: The aim of this research is to examine the effect of uncertainty in illness situations of patients receiving hemodialysis treatment on home care needs. Material and Method: This research, designed in a descriptive and cross-sectional type, was conducted between January and June 2023 with 158 patients who agreed to participate in the hemodialysis treatment study in a province of Sinop. The data of the research were collected face to face after the 'Descriptive Information Form', 'Survey for Determining Home Care Needs of Patients Receiving Hemodialysis Treatment' and 'Mishel Disease Uncertainty Scale-Society Form'. In the analysis of the data, independent sample t test, analysis of variance, person analysis and logistic regression analysis were used along with descriptive statistics. Within the scope of the research, ethics committee, institutional permission and everyone's consent were obtained. Result: Of the participants, 74.1% are involved in work related to hemodialysis, 51.3% experience problems in fulfilling family relationships and roles, and 88% do not work. When the participants' situations regarding home care were examined, it was found that 71.5% had someone responsible for home care, and 78.5% had not received any education regarding home care for themselves or their caregivers. The participants' Uncertainty in Illness Scale total score was determined to be 58.96±9.60. According to hemodialysis treatment, 58.9% of patients need assistance with shopping, 52.5% need assistance with traveling, and 65.8% need assistance when experiencing fatigue and weakness. Among home care needs, the need for personal hygiene, fluid restriction, changing clothes, and movement inside or outside the house was found to be the most significant predictor of disease uncertainty (p<0.05). It was found that those who had sufficient knowledge about their disease experienced less uncertainty (p<0.05). Conclusion: As a result of the study; It has been determined that patients receiving hemodialysis treatment experience had a moderate level of disease uncertainty, and the needs determined regarding their disease and personal needs have a predictive effect on disease uncertainty. It is recommended that patients receiving dialysis treatment be given more information about the process and supported in line with their home care needs.
