Periodontitislilerde Dişeti Küretajının El Aletleri ve Lazerle Yapılmasının Dişeti Oluğu Sıvısında Thrombomodulin Değerlerine Etkisi

Abstract

Bu çalışmanın amacı, orta şiddette periodontitisli bireylerde dişeti küretajının el aletleri ve Nd-YAG lazerle yapılmasının dişeti oluğu sıvısında (DOS) thrombomodulin (TM) değerlerine etkisinin kıyaslamalı olarak incelenmesidir.Çalışmamızda 8'i kadın 24'ü erkek olmak üzere toplam 32 adet orta şiddette periodontitisli sistemik açıdan sağlıklı bireyin, göllenmiş DOS'ndaki TM düzeyleri tedavi öncesi ve tedavi sonrası 14. günde örneklemeli olarak saptanmıştır. Aynı hastalara el aletleri grubunda diş taşı temizliği ve küretaj uygulanırken, Nd-YAG lazer grubunda diş taşı temizliğini takiben 2W 20Hz SP modunda 300?m fiber uç kullanılarak Nd-YAG lazer uç kullanılarak yumuşak doku küretajı ve kök yüzey detoksifikasyonu yapılmıştır. Tedavinin sonundaki 14. günde dişlerden tekrar DOS örnekleri toplandı ve ELISA yöntemi ile TM düzeyleri tespit edildi.Başlangıç tedavi öncesi TM konsantrasyon değerleri el aletleri grubunda 97,0±40,8 ng/ml, lazer grubunda 98.9±45.0 ng/ml iken tedavi sonrası el aletleri grubunda 90,9±49,7ng/ml, lazer grubunda 79.9±39.7 ng/ml olduğu görüldü. Lazer grubunda tedavi öncesi ve sonrası arasındaki bu farkın istatistiksel olarak anlamlı olduğu saptandı.Sonuçlar Nd-YAG lazerle yapılan kök yüzeyi detoksifikasyon ve yumuşak doku küretajının uygun bir tedavi modeli olabileceğini göstermektedir. Bulgularımız ayrıca DOS'ndaki TM seviyesi periodontal hastalık aktivitesini değerlendirmede kullanılabileceğini göstermektedir.

The aim of this study is to evaluate the effect of subgingival currettage done with hand instruments and with Nd-YAG laser in manner of monitoring the gingival crevicular fluid (GCF) thrombomodulin (TM) levels in moderate periodontitis subjects.In our study 32 subjects included in which 8 of them were female and 24 of them were male. All subjects were suffered from moderate periodontitis and otherwise healthy. TM levels in GCF were analyzed before treatment and after the 14th day of gingival currettage. In the same subjects in one side scaling and subgingival currettage and root planning were done with hand instruments whereas in the other side after scaling with hand instruments gingival currettage and root surface detoxification were applied with 2W 20 Hz SP 300?m Nd-YAG laser. In the 14th day after the treatment GCF again was collected from the same region and thrombomodulin levels were determined with ELISA technique.In the both before treatment groups the TM concentration levels were 97,0±40,8 ng/ml for hand instruments and 98.9±45.0 ng/ml in the laser groups and did not show any statistically difference. After the treatment it was found 90.9±49.7 ng/ml for hand instruments and 79.9±39.7 ng/ml for laser group. The difference between before and after the laser treatment showed statistically significant difference.The results showed that the root surface detoxification and soft tissue curretage may be a suitable treatment model in moderate periodontitis subjects. Furthermore it is thought that in GCF TM levels may be accounted on monitoring the disease activity in periodontal disease.
