Publication: Yenidoğan Yoğun Bakım Hemşirelerinin Duygusal Emek Düzeylerinin Palyatif Bakıma Yönelik Tutumlarına Etkisi
Abstract
Amaç: Bu araştırmanın amacı yenidoğan yoğun bakım hemşirelerinin duygusal emek düzeylerinin palyatif bakıma yönelik tutumlarına etkisini belirlemektir. Materyal ve Metot: Araştırmanın evrenini Aralık 2019-Ocak 2020 tarihleri arasında il merkezinde bir üniversite bir de devlet hastanesindeki yenidoğan yoğun bakım ünitelerinde (YYBÜ) çalışan toplam 96 hemşire oluşturmaktadır. Bu kapsamda araştırma kriterlerine uyan ve gönüllü olan 75 hemşire örneklemde yer almaktadır. Veriler araştırmacı tarafından literatür doğrultusunda hazırlanan tanıtıcı bilgi formu, Duygusal Emek Ölçeği (DEÖ) ve Yenidoğan Palyatif Bakım Tutum Ölçeği (YPBTÖ) kullanılarak toplanmıştır. Veriler SPSS 23 ile analiz edilmiştir. Hemşirelerin sosyo-demografik özellikleri, mesleğe bağlı değişkenleri, mesleki memnuniyet ve yenidoğan ve yenidoğan palyatif bakımına ilişkin eğitim özellikleri sayılar ve yüzdelik olarak gösterilmiştir. Veriler normal dağılım göstermediğinden ikili gruba göre nicel değişkenlerin karşılaştırılmasında Mann Whitney-U testi (U), ikiden fazla grup arasındaki fark ise Kruskal-Wallis testi (χ^2 ) kullanılarak yapılmıştır. İki değişken arasındaki korelasyon analizinde ise Spearman'ın sıra korelasyonu kullanılmıştır. Anlamlılık düzeyi p<0,05 olarak alınmıştır. Bulgular: Araştırmaya katılan YYBÜ hemşirelerinin %36'sının 31-35 yaş arasında, %68'inin evli ve %77,3'ünün lisans mezunu olduğu saptanmıştır. Hemşirelerin DEÖ alt boyutlarının puan ortalamaları sırasıyla gerçek duyguları bastırma 3,020±0,836, yüzeysel davranış 2,138±0,678, derinlemesine davranış 2,087±0,934 ve duygusal çaba harcama 1,967±0,679 şeklinde olduğu bulunmuştur. YPBTÖ alt boyutlarının puan ortalamaları ise sırasıyla organizasyon 3,317±0,501, klinik 2,350±0,558 ve kaynaklar 2,349±0,809 şeklinde olduğu saptanmıştır. Sonuç: Araştırma kapsamında hemşirelerin DEÖ'nün yüzeysel davranış alt boyutu ile YPBTÖ'nün kaynaklar alt boyutu arasında negatif yönde, DEÖ'nün gerçek duyguları bastırma alt boyutu ile YPBTÖ'nün klinik alt boyutu arasında ise pozitif yönde istatistiksel olarak anlamlı bir ilişki olduğu sonucuna varılmıştır. Aim: The aim of this study is to determine the effect of emotional labor levels of newborn intensive care nurses on their attitudes for palliative care. Material and Method: The population consists of 96 nurses in total working in newborn intensive care units (NICU) in a university and also in a state hospital in the city center between December 2019 and January 2020. In this context, 75 volunteer nurses who are eligible with the research criteria are included in the sampling. The data were collected by using the introductory information form prepared by the researcher in line with the literature, the Emotional Labor Scale (ELS) and the Newborn Palliative Care Attitude Scale (NPCAS). Data were analyzed with SPSS 23. The socio-demographic characteristics of the nurses, variables related to the profession, professional satisfaction, and educational characteristics of newborn and newborn palliative care are shown as numbers and percentages. Since the data does not demonstrate a normal distribution, Mann Whitney-U test (U) has been used to compare quantitative variables according to the binary group, and the difference between more than two groups was made by using the Kruskal-Wallis test (χ^2 ). Spearman's rank correlation has been used in the correlation analysis between two variables. The level of significance was taken as p<0.05. Results: It has been identified that 36% of the NPCAS nurses participating in the study is between 31-35 years old, and 68% of them is married and 77.3% of them has bachelor's degree. Average scores of ELS sub-dimensions of the nurses are found respectively suppression of real emotions 3,020±0,836, superficial behaviour 2,138±0,678, in depth behaviour 2,087±0,934 and emotional effort 1,967±0,679. Score averages of NPCAS sub-dimensions have been determined as organization 3,317±0,501, clinic 2,350±0,558 and resources 2,349±0,809, respectively. Conclusion: Within the scope of the study, a conclusion is reached that there is a negative statistically significant relationship between the superficial behaviour sub-dimension of the nurses' ELS and resources sub-dimension of NPCAS, and a positive statistically important relationship between suppression of real emotions sub-dimension of ELS and clinic sub-dimension of NPCAS.
