Publication: Metal Astar (Primer) Uygulamanın Ortodontik Braketlerin Bond Kuvveti Üzerine Etkisi Bir İn Vitro Çalışma
Abstract
Amaç: Ortodontik tedavi sırasında, braket taban yapısı ve mine yüzeyi arasında elde edilen bağlanma kuvvetini - özellikle braket tabanı ve adeziv rezin ara yüzeyinde - optimum seviyeye çıkarabilecek bir bağlanma ajanının (metal astar) in vitro olarak değerlendirilmesidir. Materyal ve Metot: 132 adet ortodontik tedavi amaçlı çekilmiş üst küçük azı dişi, test sürecine kadar 0,1 % thymol solüsyonda muhafaza edilmiş olup, M ve İ olmak üzere iki ana gruba ayrılmıştır. M grubunda mesh taban yapılı (3M Unitek, Victory) metal braket, İ grubunda ise integral taban yapılı (Adenta, Crown) braket kullanılmıştır. M ve İ gruplarının her ikisi 3 alt gruba ayrılıp toplamda 6 alt grup oluşturulmuştur (n = 22). MKONT ve İKONT kontrol gruplarında metal astar kullanılmadan yapıştırma işlemleri gerçekleştirilirken, MMAT ve İMAT gruplarında braket tabanının yapıştırma yüzeyinin tamamına, MMAK ve İMAK'da ise braket tabanının sadece merkezine metal astar (Reliance Metal Primer) sürülüp, yapıştırma işlemleri gerçekleştirilmiştir. Bond kuvvetinin ölçüldüğü testler Lloyd Instruments Plc cihazı ile gerçekleştirilmiştir. Dişlerin bukkal yüzeylerine braketler yapıştırılmadan önce, kopma testi sonrası ve dişte kalan adeziv maddelerinin temizliği yapıldıktan sonra, x10 büyütme Stereo-Mikroskop ile görüntüleri alınmıştır. Veriler topladıktan sonra, makaslama bağlanma dayanımı (SBS), artık adeziv indeksi (ARI) ve Stereo-Mikroskopik düzeydeki mine hasar indeksi (EDI) uygulanmıştır. Bulgular: SBS ortalaması; MKONT grupta 15,67(±7,85) MPa, MMAT grupta 18,37(±7) MPa, MMAK grupta 20,89(±4,99) MPa, İKONT grupta 13,04(±5,06) MPa, İMAT grupta 17,1(±4,28) MPa ve İMAK grupta 12,66(±4,64) MPa olarak bulunmuştur. Çift yönlü ANOVA testine göre, gruplar arasında anlamlı bir farklılık ortaya çıkmıştır (p = 0.003). ARI skoru incelemesinde Ki-kare testine göre, önemli bir fark bulunurken (p = 0.01), EDI değerlendirmelerinde anlamlı bir fark bulunmamıştır (P = 0.75). Sonuç: Metal astar uygulamasının, ortodontik braketlerin yapışma kuvvetini artırdığı tespit edilmiştir. Mesh taban yapılı braketlerin daha yüksek SBS değeri sağlamasına karşın, İntegral taban yapılı braketlerin, mine bütünlüğünü koruması nedeni ile uygulamada tercih edilebileceği kanaatine varılmıştır. Materials and methods: 132 extracted upper premolars for orthodontic treatment kept in 0.1% thymol solution until testing. The teeth divided into two main groups, M mesh-based brackets (3M Unitek, Victory) and I integral-based brackets (Adenta, Crown) metal brackets. M and I groups were divided into 3 subgroups (n=22). While the brackets in MKONT and IKONT groups has bonded without application of metal primer as control group, in MMAT and IMAT group, the metal primer (Reliance Metal Primer) was applied to the whole surface of the bracket base and in MMAK and IMAK groups, the metal primer applied to the central part of the bracket base. The brackets' debonding carried out using (Lloyd Instruments Plc) device. The buccal surfaces of the teeth were captured with x10 magnification Stereo-Microscope before brackets' bonding, after debonding and after adhesive remnants' cleaning. Shear bond strength (SBS), adhesive remnant index (ARI) and Stereo-Microscope observation of enamel damage index (EDI) were investigated. Results: The mean (Sd.) shear bond strengh for group MKONT was 15.67 (±7,85) MPa, for MMAT was 18.37 (±7) MPa , for MMAK was 20.89 (±4.99) MPa , for IKONT 13.04 (±5.06) MPa was,for IMAT was 17.1 (±4.28) MPa and for IMAK was 12.66 (±4.64) MPa . The two way ANOVA test showed a significant difference (p=0.003) between groups. The Chi-square test for ARI indicated that there was a significant difference (P=0.01) but for EDI there was no significant difference (P=0.75). Conclusion: Application of metal primer increases the bond strength of orthodontic brackets. löAlthough the mesh base brackets providing higher SBS values integral base brackets are preferred for preserving enamel integrity.
