Publication: Antiepileptik İlaçların Serum ve Saçta Eser Element Düzeyleri Üzerine Etkileri
Abstract
ÖZET Epileptik hastalarda valproik asid ve fenobarbital tedavisinin serum ve saçta eser element düzeyleri üzerine etkilerini göstermek amacıyla Ondokuz Mayıs Üniversitesi Tıp Fakültesi Çocuk Nörolojisi Bilim Dalında klinik ve laboratuvar incelemelerle epilepsi tanısı alan 40 çocuk incelendi. Hastaların 20' sine fenobarbital ve kalan 20' sine ise valproik asid tedavisi başlandı. Her iki grubta da serumda Zn, Cu, Mg düzeyleri ile saçta Zn düzeyleri ölçüldü. Bu ölçümler antiepileptik tedavi(fenobarbital veya valproik asid) başlanmadan önce ve tedavi başlandıktan üç ay sonra yapıldı. Hem VPA hem de FB tedavisiyle epileptik hastalarda serum Zn konsantrasyonlarının azaldığı, serum Cu, Mg ve saç Zn düzeylerinde değişiklik olmadığı saptandı. Hastaların kan FB, VPA düzeyleriyle serum ve saçta eser element düzeyleri arasında anlamlı bir ilişki saptanmadı. Sonuç olarak serum çinko miktarının total vücut çinkosunun ancak %1'ini oluşturduğu ve antiepileptik ilaçların eser elementlerin intraselüler dağılımını etkilediği düşünülecek olursa, serum çinko konsantrasyonu normal olsa bile çinko eksikliği ile ilgili klinik bulguların ortaya çıkabileceği düşünülmelidir. Bu nedenle hekim, uzun süre antiepileptik ilaç alan hastalarda eser elementlerin eksikliğine bağlı olarak yan etkiler ortaya çıkabileceğinden dolayı dikkatli olmalı ve gerektiğinde replasman tedavisini düşünmelidir.
SUMMARY Forty patients that have clinical and biochemical diagnosis of epilepsy was examined to show the effect of Valproic acid and Phenobarbital treatment on the trace elements in serum and hair in Pediatric Neurology Department of Ondokuz Mayıs University. Phenobarbital was given to twenty patients and valproic acid was given to the other twenty patients. Plasma zinc, copper, magnesium levels and zinc levels in hair were measured at both groups. These measurements were made before the treatment and three months after the beginning of the treatment. In epileptic patients both valproic acid and phenobarbital treatment plasma zinc level is decreased. Plasma copper, magnesium and hair zinc level isn't changed. These changes aren't meaningful and relation between blood phenobarbital and valproic acid and trace elements aren't meaningful. In conclusion we know that plasma zinc levels is 1 percent of total body zinc amount and the antiepileptic drugs affects of the trace elements in intracellular distribution so that we must think clinic signs of zinc defiency although plasma zinc concentration is normal. So the physician should aware that the long usage of antiepileptic drugs could result in the trace element deficiency, so in selected cases the replacement therapy could be started.
