Abstract
Amaç: HT oluşturuğu hedef organ hasarları ile dünyada mortalite ve morbiditenin önde gelen sebeplerindendir. Endotel disfonksiyonu ile HT oldukça ilişkilidir ve HT da gözlenen vasküler tonus artışının temelde endotel tarafından salgılanan vazodilatatör ve vazokonstriktör mediyatörler arasındaki dengesizlikten kaynaklandığı öne sürülmüştür. Bu çalışmamızda Esansiyel hipertansiyonu olan hastaların; arteriyel tansiyon düzeyi ile endotel fonksiyonlarının değerlendirilmesinde kullanılan Aortik 'Stiffness' ve serum Endotelin-1 ile olan ilişkisinin değerlendirilmesi amaçlanmıştır.Yöntem: Prospektif olarak Ondokuz Mayıs Üniversitesi Tıp Fakültesi Kardiyoloji Anabilim Dalında Ocak 2010 ile Ocak 2011 tarihleri arasında başvuran ve seçilme kriterlerini karşılayan; en az 1 yıl önce esansiyel hipertansiyon tanısı almış, 18-70 yaş arası hastalar ve kontrol grubu olarak da 49 sağlıklı bireyler olmak üzere toplam 152 olgu çalışmaya dahil edildi. HT tanılı hastalar uygun ölçüm yöntemleri ile bir hafta boyunca evde KB ölçümü yaptı. Hastalar TA ortalamalarına göre regüle hipertansif(47 kişi) ve disregüle hipertansif (56 kişi) olmak üzere iki gruba ayrıldı. Kontrol grubu ile birlikte toplam üç gruba da ekokardiografi yapılarak Aortun elastik parametreleri(aortik strain,distensibilite,aortik stiffness index) hesaplandı. Kan örnekleri alınarak serum ET-1 düzeyleri ELİZA yöntemi ile çalışıldı.Bulgular: Çalışmaya alınan tüm deneklerin serum ET-1 düzeyleri ile aortun elastik parametreleri arasında anlamlı bir ilişki gözlendi(p=0.004) TA düzeyi ile aortun elastik parametreleri arasındaki incelendiğinde ise; disregüle hipertansiyonu olan grubun aortun elastik parametreleri yalnızca kontrol grubundan değil aynı zamanda regüle hipertanif hasta grubundan da belirgin olarak farklıydı.(p<0.01) ET-1 düzeyi her üç grupta da farklı olmasına rağmen sonuçlar istatiksel olarak anlamlı değildi.(p=0.101)Sonuç: Çalışmamıza katılan tüm hastaların endotelin-1 düzeyleri ile aortun elastik parametreleri arasında anlamlı bir ilişki saptanmıştır. TA düzeyi ile aortun elastik parametreleri arasındaki ilişki incelendiğinde ise; aortun elastik parametrelerinin her üç grupta da farklı oluşu aortik sertliğin TA yüksekliği ile ilişkili olduğunu ve kullanılan antihipertansif ilaçlar ile sağlanan TA regülasyonun aortik sertliği düzeltebileceğini göstermektedir. Bu durum günlük pratikte hipertansif hastalarda ölçülen aortun elastik parametrelerinin tansiyon regülasyonu hakkında fikir sağlayabileceğini düşündürtmektedir
Objective: Hypertension is the leading cause of mortality and morbidity in the world as a result of the target organ damage. Endothelial dysfunction is highly associated with hypertension. It has been hypothesized that imbalance between vasodilator and vasoconstrictor mediators secreted by endothelium resulted in increase in vascular tonus. In this study; we aimed to evaluate the relationship between serum endothelin-1 and aortic stiffness that used for assessment of endothelial function and arterial blood pressure level in patients with essential hypertension.Method: In this prospective study; 49 healthy subjects and 103 patients who admitted to Ondokuz Mayıs University, Faculty of Medicine, Cardiology Department between January 2010 and January 2011, appropriate for inclusion criterias, diagnosed as essential hypertension at least one year ago and aged between 18 and 70 years-old were included. The patients with essential hypertension measured their blood pressure levels with approved digital automatic blood pressure monitors at home during a week. Patients were divided into regular (47) and dysregular (56) hypertensive groups according to average blood pressure levels. Transthoracic echocardiographic assessments to calculate aortic elastic properties (aortic strain, distensibility, aortic stiffness index) were performed in 3 groups including the healthy control group. Serum ET-1 levels were measured by ELISA method from the blood samples obtained from all 3 groups.Results: There was a significant correlation between serum endothelin-1 levels and aortic elastic properties of all volunteers in this study (p=0,004). When the relationship between blood pressure levels and aortic elastic properties were assessed; aortic elastic properties of dysregular hypertensive group were obviously different from not only healthy control group but also regular hypertensive group (p<0,01). Altough ET-1 levels were different in 3 groups, but this result was not the statistically significant (p=0,101).Conclusion: The significant relationship was determined between aortic elastic properties and endothelin-1 levels of all volunteers included in this study. When the relationship between blood pressure levels and aortic elastic properties were assessed; the difference between all 3 groups regarding aortic elastic properties might indicate that aortic stiffness was related to high blood pressure levels and blood pressure control by antihypertensive drugs could improve the aortic stiffness. These results make us to think that in daily practice; measurement aortic elastic properties in hypertensive patients might give an idea of blood pressure level regulation.
