Transizyonel Hücreli Mesane Kanserlerinde İntravezikal İmmüno

Abstract

ÖZET Mesane tümörlerinde marker olarak kullanılan NMP22 testinin spesifitesi pek çok faktör tarafından etkilenmektedir. İntravezikal immüno/kemoterapinin, NMP22 düzeyine etkisini ortaya koymak ve optimal bir eşik değeri belirlemek üzere bu çalışmayı planladık. Yüzeyel mesane kanseri tanısıyla intravezikal BCG tedavisi uygulanmış' veya uygulanmakta olan 41 ve intravezikal kemoterapi alan 29 hastanın idrar örneklerinde NMP22 düzeyleri ölçüldü. Ortalama NMP22 düzeyi, intravezikal BCG almakta olan grupta 39.9 (3- 103) U/ml, intravezikal BCG almış grupta 35.9 (5-130) U/ml ve intravezikal kemoterapi almış grupta ise 22.3 (2-120) U/ml olarak bulundu. Gruplar arasında NMP22 ortalama değerlerinde istatiksiksel olarak anlamlı fark saptanmadı (p>0.05). İntravezikal kemoterapi almış hastalarda testin sensitivite, spesiflte, pozitif prediktif değer ve negatif prediktif değerleri sırasıyla %100, %42.8, %5.8 ve %100, intravezikal BCG almakta olan hastalarda %33.3, %10, %18 ve %20, intravezikal BCG almış hastalarda ise %66.6, %22.7, %10.5 ve %83.3 olarak bulunmuştur. ROC eğrisi ile saptanan yeni eşik değer 13 U/ml olarak alındığında testin spesifitesi intravezikal BCG almakta olan hastalarda %33.3'e ve intravezikal BCG almış hastalarda %40.9'a yükseldi. Buna karşılık yeni değer testin sensitivitesini düşürmedi. Sonuç olarak, klinik uygulama kolaylığı açısından intravezikal BCG tedavi öyküsü olanlarda NMP22 testinin eşik değerinin 13 U/ml olarak alınması daha faydalı olacaktır. Bu uygulama testin sensitivitesinde değişiklik olmaksızın spesifitesini artırmaktadır. Başka bir deyişle alınan bu yeni eşik değer testin yalancı negatiflik oranını değiştirmeksizin, yalancı pozitiflik oranını düşürmektedir. ABSTRACT It has been demonstrated that many factors decrease the efficacy of the NMP22 test especially of its specifity. In this study we searched for the effect of intravesical irnmuno/chemotherapy on NMP22 levels and investigated for a new cut off value especially for this group of patients to overwhelm the effect. Urine specimens collected from 41 patients treated intravesical BCG and 29 patients treated with intravesical chemotherapy for superficial bladder cancer were analysed to document the NMP22 levels. The mean value of NMP22 levels were found to be 39.9 (3-103) U/ml in patients on BCG treatment, 35.9 (5-130) U/ml in patients previously treated with BCG and 22.3 (2-120) U/ml in patients treated with intravesical chemotherapy. The mean NMP22 levels did not differ significantly between the groups (p>0.05). The sensitivity, specifity, positive predictive value and negative predictive value were 100%, 42.8%, 5.8% and 100% respectively in patients treated with intravesical chemotherapy, the same parameters were found to be %33.3, %10, 18% and 20% respectively in the patients on BCG treatment, and 66.6%, 22.7%, 10.5% and 83.3 % respectively in patients previously treated with BCG. The specifity of the test was increased to 33.3% in patients on BCG treatment and to 40.9% in patients previously treated with BCG with a new cut off value of 13U/ml from ROC curves. This new value did not lower the sensitivity of the test. As a conclusion, it would be useful! clinically to set the cut off value to 13U/ml for patients with a history of intravesical BCG treatment. This would increase the specifity of the test without any effect on sensitivity. In other words this new value would decrease the false positive results without any effect on false negatives.
