Sigara Kullanan ve Kullanmayan Generalize Kronik Periodontitisli Hastalarda Faz I Periodontal Tedavi Öncesi ve 6 Ay Sonrası Ve-cadherin ve Vegf Seviyelerinin Araştırılması

Abstract

Amaç: Çalışmamızda; sigara kullanan ve kullanmayan generalize periodontitisli bireylere geleneksel cerrahi olmayan Faz I tedavi tüm-ağız debridman (TAD) protokolüyle gerçekleştirilerek, tedaviden 6 ay sonra ve tedavi öncesi iyileşme durumları kıyaslanmıştır. Sigaranın bu tedavi protokolünün etkinliğine yansımasını görebilmek için dişeti oluğu sıvısı (DOS) VEGF ve VE-cadherin düzeylerinin tespiti yapılmıştır. Materyal ve Metot: Çalışmaya sistemik sağlıklı 11 sigara kullanan (grup 1), 12 sigara kullanmayan (grup 2) generalize kronik peridontitisli, 13 sigara kullanan (grup 3) ve 13 sigara kullanmayan (grup 4) sağlıklı hasta dahil edildi ve generalize kronik periodontitisli gruplara TAD tedavisi uygulandı. Tedavi öncesi ve tedavi sonrası 6. ayda periodontal durumu değerlendiren klinik ölçümler yapıldı ve en derin cep derinliğine sahip 2 diş bölgesinden DOS örnekleri toplandı. ELISA yöntemiyle DOS VE-cadherin ve VEGF seviyeleri tespit edilerek grup içi ve gruplar arası değerlendirmeler gerçekleştirildi. Toplanan klinik veriler ve DOS hacimleri de çalışma parametreleri olarak kullanıldı. Bulgular: TAD tedavisi sonrası klinik parametreler ve DOS hacminde tüm gruplarda düşüş görüldü. Grup 1 ve grup 2 de VEGF total miktarları tedavi sonrası anlamlı düşük bulundu (p<0,05). Her iki grupta da tedavi sonrası total VE-cadherin miktarlarında başlangıca göre fark bulunmadı. Sonuç: Bulgularımız, sigara kullanan ve kullanmayan bireylerde TAD tedavisi sonucu periodontal iyileşmenin geçirgenlik ve damarlanmada azalış yönünde olacağı fikrini desteklemektedir. Aim: In our study, traditional non-surgical Phase I treatment for individuals with and without smoking generalized periodontitis was performed with the full-mouth debridement (FMD) protocol and their recovery status was compared 6 months after treatment and before treatment. In order to see the reflection of smoking on the effectiveness of this treatment protocol, gingival crevicular fluid (GCF) VEGF and VE- cadherin levels were determined. Material and Method: The study included 13 smokers with periodontitis (group 1), 13 non-smokers with periodontitis (group 2), 13 smokers without periodontitis (group 3) and 13 non-smokers without periodontitis, all of whom are systemically healthy male patients and FMD treatment was applied to generalized chronic periodontitis groups. Clinical measurements were made before and after (6 months) treatment and gingival crevicular fluid (GCF) samples were collected from two teeth with the deepest pocket depths. VE- cadherin and VEGF levels were determined in GCF samples by ELISA and intra-/inter group assessments were done. The collected clinical data and GCF volumes were also utilized as the study parameters. Results: After TAD treatment, clinical parameters and DOS volume decreased in all groups. Total amounts were found to be significantly lower after treatment (p <0.05). No difference was found in the total VE-cadherin amounts from baseline. Conclusions: Our findings support the idea that periodontal healing will decrease in permeability and vascularity as a result of TAD treatment in smokers and non-smokers.
