Parkinson Hastalarında Yürüme Sırasındaki Donma Sıklığı İle Rem Uyku Davranış Bozukluğunun Şiddeti Arasındaki İlişki

Abstract

Giriş ve Amaç: İdiyopatik Parkinson Hastalığı'nda (iPH) yürüyüş sırasında donma(YSD) ve Rem uyku davranış bozukluğu(RUDB) seyrek değildir. Yapılan çalışmalarda İPH'da, hem YSD hem de RUDB'nin patofizyolojisinde Pedinkülopontin nükleus'un (PPN) tutulduğunun gösterilmesi bu iki durum arasında bir ilişkinin olabileceğini düşündürmektedir. Bu nedenle biz çalışmamızda Parkinson hastalarında RUDB'nin klinik şiddeti ve YSD arasında bir ilişki olup olmadığını belirlemeyi amaçladık. Gereç ve Yöntem: Çalışmaya YSD ve klinik muhtemel RUDB'si olan toplam 33 İPH hastası alındı. Hastaların evrelemesi on döneminde Hoehn and Yahr evreleme skalasına (HYE) göre, motor belirtilerin şiddeti on döneminde UPDRS III'e göre yapıldı. Hastaların ON ve OFF dönemlerinde YSD sıklığı/şiddetini ölçmek için UPDRS II bölüm 14 testi kullanıldı. Klinik muhtemel RUDB'nin şiddeti RUDB değerlendirme anketine (RDBSQ) göre belirlendi. Ayrıca hastaların kategori-leksikal akıcılıkla birlikte diğer yürütücü fonksiyon testleri yapıldı. Çalışmada esas olarak hastaların on ve off dönemlerindeki YSD skorlarının RDBSQ skorları ile korelasyonu test edildi. İlave olarak YSD skorları; HYE skorları ve akıcılık test skorları ile karşılaştırıldı. Bulgular: YSD on dönemi skoru ortalama 0,8±1,03,YSD off dönemi skoru ortalama 2,3±0,73 idi. Tüm hastaların RDBSQ anketi ortalama puanı 7,1±2,06 idi. Hastaların on ve off dönemlerindeki YSD skorları ile akıcılık testleri ve RDBSQ skorları arasında korelasyon saptanmadı. Sonuç: Çalışmamızda klinik RUDB ile YSD sıklığı arasında anlamlı bir ilişki bulamadık. (Hem YSD ON hem de YSD OFF, RUDB skorları ile ilişkili değildi). Bu bulgular her iki durumun patofizyolojik mekanizmalarında rol alan yapılardan ortak olanlar(Örn. PPN) yanında ortak olmayanların da çok önemli olduğunu göstermektedir. İlave olarak bu iki durumdan birinin klinik şiddetinin derecesi diğerini anlamlı olarak etkilemeyeceği söylenebilir.

Introduction and objective: During Idiopathic Parkinson's disease (IPD), freezing of gait (FOG) and REM sleep behavior disorder (RBD) are not uncommon. Studies show that; in IPD the pathophysiology of both FOG and RBD in the Pedinkülopontin nucleus (PPN), suggests that there may be a relationship between these two conditions. For this reason, in our study we aimed to determine whether there is a relationship between clinical severity of RBD and FOG in Parkinson's disease. Materials and Methods: A total of 33 IPD patients were included in the study, they have FOG and clinically probable RBD. Staging of patients performed according to on state Hoehn and Yahr staging scale (HY), severity of the motor symptoms according to on state UPDRS III. UPDRS II part 14 test was used to staging the frequency / severity of FOG in ON state and OFF state periods of patients. Clinical probable RBD severity was determined according to the RBD screening questionnaire (RBDSQ). In addition, we performed lexical category test and other executive function performance test. In this study, the correlation of the FOG scores with the RBDSQ scores in the on state and off state periods of the patients was tested. In addition, FOG scores are compared with HY scores and lexical category scores. Results: The mean scores of the FOG on state was 0.8 ± 1.03 and the mean scores of the FOG off state was 2.3 ± 0.73. The mean score of all patients in the RBDSQ was 7.1 ± 2.06. There was no correlation between FOG scores in the on state and off state periods of patients and lexical category tests and RBDSQ scores. Conclusion: In our study, we could not find any significant relationship between clinical RBD and FOG frequency. (Both FOG ON state and FOG OF state were not associated with RBD scores). These findings show that; pathophysiological mechanisms of both situations have parter pathways (e.g. PPN) but it is important that they have non-partner pathways. In addition, we can say that the clinical severity of one of these two conditions will not significantly affect the other one.
