Abstract
Diyabetli ve sistemik sağlıklı bireylerde indüklenebilir Nitrik Oksit Sentaz (iNOS) ve proinflamatuvar medyatörlerin seviyesinin, sağlıklı ve inflamasyonlu dişetlerinde kıyaslamalı olarak değerlendirilmesi ve diyabetin kontrol derecesi ile inflamatuvar olaylar arasındaki ilişkinin incelemesi amaçlandı.Çalışmamızın deney ve kontrol grubu, toplam 118 hastadan oluşturuldu. Çalışmamıza katılan bireyler, periodontal durum ve diyabetin metabolik kontrol durumlarına göre değerlendirilerek altı gruba ayrıldı.Hastalarımızın yapışık dişeti bölgelerinden elde edilen diş eti biyopsilerinin homojenizasyonu sonucu elde edilen süpernatantlarında prostaglandin E2 (PGE2), interlökin 1 beta (IL-1ß), tümör nekroz faktör alfa (TNF-?), indüklenebilir nitrik oksit sentaz (iNOS) ve interferon gamma (IFN-?) seviyeleri ELISA testleri ile tespit edildi.Kontrolsüz diyabetli ve periodontitisli örneklerdeki proinflamatuvar medyatörlerin seviyesi en yüksek bulunmuştur (p<0,05). Kontrollü diyabetli ve periodontitisli örneklerde IL-1ß dışında tüm medyatörler sağlıklı periodontitisli örneklerle benzer bulunmuştur (p>0,05). Sağlıklı dokular değerlendirildiğinde ise kontrolsüz diyabetlilerin örneklerindeki değerler, sağlıklı bireylerle ve kontrollü diyabetlilerle benzer bulunmuştur (p>0,05).Çalışmamızda, metabolik kontrolü zayıf olan diyabetiklerde inflamatuvar yanıtın arttığı, metabolik kontrolü iyi olan diyabetiklerde ise sistemik sağlıklılarla benzer inflamatuvar yanıtın olduğu gözlendi.
The purpose of our study is to evaluate the gingival tissue levels of inducible nitric oxide synthase (iNOS) between healthy and inflamed gingiva and to examine the relationship between inflammatory events and control degree of diabetes mellitus.The study and control groups of our study are consisted of 118 patients totally. Participants were classified to six groups according to the evaluation of their periodontal status and metabolic conditions of diabetes mellitus.The prostaglandin E2 (PGE2), interleukin 1 beta (IL-1ß), tumor necrosis factor alpha (TNF-?), inducible nitric oxide synthase (iNOS) ve interferron gamma (IFN-?) ELISA tests were applied to the supernatants which achieved from homogenization of the gingival biopsies obtained from attached gingiva of our patients.The levels of All proinflammatory mediators were highest in the patients with uncontrolled diabetes and periodontitis (p<0,05). In well controlled diabetic patients with periodontitis, all mediators except IL-1ß had similar levels with patients who were sistemically healthy but have periodontitis (p>0,05). In the evaluation of healthy tissues, the values of uncontrolled diabetic patients were found similar with both sistemically healthy patients and controlled diabetic patients (p>0,05).As a result of this study, the inflammatory response increases in patients with poor metabolic control but the inflammatory response of well controlled diabetics is similar with sistemically healhy participants.
