Abstract
Amaç: Bu çalışmada, tiroid otoantikoru pozitif olan ötiroid hastalarla sağlıklı bireylerin yaşam kalitelerinin karşılaştırılması amaçlanmıştır. Materyal ve Metod: Tiroid hastalığı semptomları ile başvuran hastaların önemli bir kısmında, yapılan tetkiklerde tiroid fonksiyon testleri normal olmasına rağmen otoantikorları yüksek bulunmaktadır. Yapılan tetkiklerde tiroid fonksiyon testleri normal, tiroid otoantikorları pozitif olanlar hasta grubuna alınmıştır (n=100). Tiroid fonksiyon testleri normal, tiroid otoantikorları negatif tespit edilenler, yani sağlıklı bireyler ise kontrol grubuna alınmıştır (n=100). Yaşam kalitesini değerlendirmek amacıyla kısa form-36 anketi (SF-36) uygulanmıştır. SF-36'nın yüksek skorları yaşam kalitesindeki iyilikle ilişkilidir. Tiroid otoantikoru pozitifliğinin yaşam kalitesi üzerine etkisinin hastada depresyon ve anksiyete varlığından etkilenip etkilenmediğini göstermek için Beck Depresyon ve Beck Anksiyete Ölçekleri kullanılmıştır. Bulgular: Tiroid otoantikoru pozitif hastalarla kontrol grubu karşılaştırıldığında SF-36'nın fiziksel fonksiyon, fiziksel rol güçlüğü, emosyonel rol güçlüğü, sosyal fonksiyon, mental sağlık, enerji, fiziksel ve mental komponent skorları hasta grubunda anlamlı olarak düşük bulunmuştur (p<0,05). Hasta ve kontrol grubu arasında yapılan karşılaştırmada Beck Depresyon ve Beck Anksiyete skorları hasta grubunda anlamlı olarak yüksek saptanmıştır (p<0,05). Hasta grubunda SF-36'nın tüm parametreleri (fiziksel fonksiyon, fiziksel rol güçlüğü, bedensel ağrı, genel sağlık algısı, emosyonel rol güçlüğü, sosyal fonksiyon, mental sağlık, enerji, fiziksel komponent skoru, mental komponent skoru) ile hem Beck Depresyon hem Beck Anksiyete skorları ile negatif yönde anlamlı ilişki saptanmıştır. Sonuç: Bu çalışmada yaşam kalitesi, tiroid otoantikoru pozitif olan, ötiroid hasta grubunda sağlıklı gönüllülerle karşılaştırıldığında daha düşük bulunmuş, depresyon ve anksiyete arttıkça yaşam kalitesinin olumsuz etkilendiği gösterilmiştir. Aim: The aim of the present study was to compare quality of life of euthyroid patients with positive thyroidautoantibodies and healthy individuals. Material and Method: The majority of patients admitted with a thyroid disease have high titers of thyroid antibodies despite normal thyroid function tests. After examinations, individuals with normal thyroid function tests and who were positive for thyroid autoantibodies were included in the patient group (n=100). Individuals with normal thyroid function tests and who were negative for thyroid autoantibodies (i.e. healthy individuals) were included in the control group (n=100). The short form-36 questionnaire (SF-36) was administered to evaluate quality of life. High SF-36 scores are associated with good quality of life. Beck Depression and Anxiety Inventories were used to determine whether the effect of thyroid autoantibody positivity on patient's quality of life is affected by the presence of depression and anxiety. Results: Physical functioning, role physical, role emotional, social functioning, mental health, vitality, physical and mental component scores were significantly lower in the patient group compared to the control group (p<0.05).Beck Depression and Anxiety Inventory scores were significantly higher in the patient group (p<0.05). In the patient group, a negative correlation was found between all SF-36 parameters (physical functioning, role physical, bodily pain, general health, role emotional, social functioning, mental health, vitality, physical component score, and mental component score) and both Beck Depression and Anxiety Inventories scores. Conclusion: The current study revealed that the quality of life of euthyroid patients with positive thyroid autoantibodies was lower compared to healthy volunteers, and demonstrated that quality of life was negatively affected with increasing depression and anxiety scores.
