Histerektomi Sonrası Kadınlarda Gelişen Osteopeninin Periodontal Durum ile İlişkisinin Değerlendirilmesi

Abstract

Bu çalışmanın amacı histerektomi ve bilateral ooferektomi yapılmış (çalışma grubu) ve perimenopozal belirtiler gösteren (kontrol grubu) bayan hastaları osteopeni/osteoporoz gelişimi açısından değerlendirmek ve bu durumun periodontal dokular ile olan ilişkisini ortaya koymak amacıyla yapılmıştır.Çalışma Samsun Ondokuz Mayıs Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Polikliniği, Radyoloji Polikliniği ve Samsun Ondokuz Mayıs Üniversitesi Diş Hekimliği Fakültesi Periodontoloji AD olmak üzere 3 farklı merkezde yürütülmüştür. Araştırma topluluğunu bilateral ooferektomi ve total histerektomi (BO+H) endikasyonu konmuş (çalışma grubu;n:29) ve perimenopozal belirtiler gösteren, histerektomi olma ihtimali bulunan (kontrol grubu;n:22) toplam 51 bayan hasta oluşturmuştur. Tüm hastalara başlangıç ve 12. ayda olmak üzere hem periodontal klinik değerlendirmeler hem de alveoler bilgisayarlı tomografi ve kan tetkiklerini içeren medikal takip yapılmıştır. Aynı zamanda tüm hastalar kemik kütle yoğunluğu açısından da değerlendirilmiştir.Cerrahi olarak menopoza girmiş bayanlar ile perimenopozal dönemdeki bayanlarda görülen osteopeninin/osteoporozun alveoler kemik üzerine etkisi geleneksel yöntemlere göre daha üstün ve hassas olan 3 boyutlu MDBT ile güvenilir bir biçimde ölçülmüştür. Çalışmamızda, çalışma grubunda femur boynu KMY ile alveoler kemik kaybı miktarı arasında istatistiksel olarak anlamlı, pozitif bir ilişki bulunmuştur.Bu çalışmada istatistiksel olarak Wilcoxon testi, Mann-Whitney-U testi, Pearson korelasyon testi ve bağımsız t-testi kullanılmıştır. Sonuçlar, anlamlılık p<0,05 düzeyinde değerlendirilmiştir.

The purpose of this study was to evaluate respect for osteopenia/osteoporosis in a group of women who underwent hysterectomy and another group of women who manifested perimenopausal symtoms and aimed to achieve the relation between this situation and periodontal status.This research was carried out in the following three centers: Department of Obstetrics and Gynecology, Department of Radiology the Faculty of Medicine and Faculty of Dentistry of Ondokuz Mayıs University. Totally 51 women included in this study; 29 of them had an indication for bilateral ooferectomy and total hysterectomy (BO+H), 22 of them shows perimenopausal symtoms and had total hysterectomy possibility. For all of the patients at the beginning and 12. month either periodontal clinical evaluation or alveolar computed tomography (CT) and a medical following including blood investigation had been done. At same time all the patients was also evaluated respect for bone mass density.The effect of osteopenia/osteoporosis on the alveolar bone was measured with MDCT which has beter properties than the other conventional methods, in women who underwent hysterectomy and who manifested perimenopausal symtoms. It is found a statictically significiant and positive relation between femur neck BMD and alveolar bone loss in the hysterectomized group.In this study Wilcoxon test, Mann-Whitney-U test, Pearson correlation test and independent t test were used. The results were evaluated at the level of significance.
