Lökositoz ve Trombositozun Endometrium Kanserini Değerlendirmedeki Yeri

Abstract

Amaç: Bu çalışmanın amacı endometrium kanseri tanısı almış hastalarda preoperatif lökosit ve trombosit sayılarının hastalık ve hastalığın yaygınlığı ile olan ilişkisini araştırmak. Gereç ve Yöntem: 19 Mayıs Üniversitesi Tıp Fakültesi Kadın hastalıkları ve Doğum Kliniği' nde 2005-2013 yılları arasında endometrium kanseri nedeniyle opere edilmiş 177 ve kontrol grubu olarak benign nedenlerle histerektomi + bilateral salpingoooferektomi yapılmış 100 hasta çalışmaya dahil edildi. Hastalar retrospektif olarak değerlendirildi. İstatistiksel analiz student's t testi, Chi-Square testi ve lojistik regresyon analiz yöntemi kullanıldı. Bulgular: Hastaların ortalama yaşı 59.6 (30-90) yıldı. Operasyon öncesi endometrium kanserli hastaların ortalama lökosit sayısı 8100/mm³ (3620-16.100±2668), trombosit sayısı 302.000/mm³(163-742±91) olarak bulundu. Kontrol grubunda ise lökosit sayısı 7039/mm³ (4270-10.690±1474), trombosit sayısı 296.000/mm³(159-543±75) olarak bulundu. Endometrium kanserli hastaların % 21' inde lökositoz, % 5.6' sında trombositoz izlendi. Bu değerler kontrol grubunda ise sırasıyla %6 ve % 3 şeklindeydi. Kanser grubu hastalar, kontrol grubu ile karşılaştırıldığında, lökosit değerleri arasında anlamlı bir fark izlenmekteyken, trombosit değerleri için anlamlı bir fark izlenmedi. Preoperatif lökosit değerleri ileri evre hastalıkta anlamlı olarak yüksek iken (p<0.05) trombosit değerlerinde anlamlı bir fark izlenmedi (p>0.05). Endometrium kanserli hastalar, lökositoz varlığına göre iki gruba ayrıldıklarında lökositoz olan gruptaki hastaların lökositozu olmayanlara göre, daha büyük bir yüzdesinde adneksiyel tutulum (24.3% ile 5.7%, crude p=0.002), lenfovasküler invazyon (32.4% ile 12.1%, crude p=0.007), pelvik lenf nodu tutulumu (27% ile 8.6%, crude p=0.006), paraaortik lenf nodu tutulumu (21.6% ile 5.7%, crude p=0.007) izlendi. Ayrıca evre 3 hastalık (24.3% ile 12.1% crude p=0.035) ve evre 4 hastalık (21.6% ile 0.7% crude p=0.001) lökositoz grubunda anlamlı olarak daha sık izlendi. Çok değişkenli analiz uygulandığında ileri evre hastalığın lökositozu etkileyen bağımsız faktör olduğu görüldü. Sonuç: Endometrial karsinomlu olgularda, lökositoz prognozu saptamada iyi bir gösterge olabilir. Anahtar Kelimeler: Endometrium kanseri, lökositoz, trombositoz

Objective: The aim of this study was to investigate the association between preoperative leukocyte and platelet counts and the extent of the disease in patients with endometrial cancer. Materials and Methods: Between 2005-2103, 177 patients that underwent surgery with the indicatıon of endometrial cancer and 100 patients that underwent hysterectomy and bilateral salpingooophorectomy for benign causes were included in this study at the Obstetrics and Gynecology Clinic of Ondokuz Mayıs University Faculty of Medicine. Patients are evaluated retrospectively. To analyze statistically used student t-test, Chi-Square test and logistic regression analyses Results: The mean age of the patients was 56.59 years (30-90). The mean of the preoperative leukocyte count among patients with endometrial cancer was 8100/mm³ (3620-16.100±2668), platelet count was 302.000/mm³ (163-742±91). The mean of the preoperative leukocyte count among patients with control group was 7039/mm³ (4270-10.690±1474), platelet count was 296.000/mm³ (159-543±75). In the group patients with endometrial cancer, preoperative leukocytosis was present in % 21, thrombocytosis in % 5.6. These values were %6 and % 3, respectively in the control group. The patients with cancer group compared with control group, there was a significant difference between leukocyte counts but there was no significant difference between platelet counts.The preoperative leukocyte values were significantly higher in the advanced stage of endometrial cancers than early stage endometrıum cancers (p<0.005), we didn't find any difference in platelet counts like this (p>0.05) When patients were separated into two groups based on the presence or absence of preoperative leukocytosis, the leukocytosis group was associated with a greater percentage of patients with adnexal involvement (24.3% vs. 5.7% crude p=0.002), lymphvascular space invasion (32.4% vs. 12.1% crude p=0.007), pelvic involvement (27% vs. 8.6% crude p=0.006) and paraaortic involvement (21.6% vs. 5.7% crude p=0.007). The leukocytosis group had also a greater percentage of patients with stage 3 (24.3% vs. 12.1% crude p=0.035) and 4 (21.6 % vs. 0.7% crude p=0.001) disease. On multivariate analysis, advanced stage of disease was found to be an independent factor affecting preoperative leukocytosis.
