Abstract
Amaç: Birinci basamakta hasta bakış açısını değerlendirmek gereklidir. Bizim amacımız Samsun'da birinci basamakta hasta memnuniyetine etki eden faktörleri ve aile hekimliği uygulamasının etkisini araştırmaktır.Yöntem: Aralık 2006 ve Eylül 2007'de EUROPEP'in Türkçe versiyonu beş farklı kırsal ve kentsel birinci basamak merkezinde hastalara uygulanmıştır. Birinci basamak hekimleri beş iş günlük süre boyunca günde iki kez ziyaret edilmiştir. Her gün birinci basamak hizmeti almaya gelmiş en az on hasta, gönüllüler için bilgilendirilmiş olur ardından çalışmaya alınmıştır. Hastalara demografik bilgileri ve 1- 5 arası (kötüden mükemmele kadar) derecelendirilen bir ölçekle EUROPEP'teki 23 soru sorulmuştur. Demografik veriler ve bulgular değerlendirilmiştir.Bulgular: 580 hastanın 389'u (% 67,1) kadın, 191'i (% 32,9) erkekti. Toplam 383 kişi erişkindi, pediatrik grup için ebeveynle görüşülmüştür. ?Acil sağlık sorunları için hızlı hizmet sunması? 2,13 ± 1,32 ile en kötü sonuç olarak değerlendirildi. ?Sizi dinlemesi? 4,45 ± 0,71 ile en iyi sonuç olarak değerlendirildi. Samsun'da Aile Hekimliği uygulaması başladıktan sonra yedi soruya verilen puan ortalamaları istatistiksel açıdan anlamlı bir şekilde yükselmişti. Bu sorular: ?Test ve tedavilerin amaçlarını açıklaması?, ?size uygun zamanlarda randevu alabilme?, ?sağlık merkezine telefonla ulaşabilme? ve ?doktorunuza telefonla ulaşabilme? , ?acil sağlık sorunları için hızlı hizmet sunumu? , ?uzmana uygun şekilde sevk? ve ?bekleme odasında harcanan zaman?. Beş soruya verilen yanıtların puan ortalamaları aile hekimliği uygulaması sonrasında istatistiksel açıdan anlamlı bir şekilde azalmıştı. Aim: Evaluation of patient perspective is necessary in primary care. Our aim was to explore the factors and practice of family practice on the patient satisfaction on primary health care in Samsun.Methods: Turkish version of the EUROPEP was administered to patients in five different rural and urban primary health care centers in December 2006 and September 2007. Primary care physicians were visited for the study in five day medical practices, twice for each day. Every day, a minimum of 10 patients who visited primary care, were consecutively included the study after the informed consent. The patients were asked about demographic details and 23 questions in EUROPEP with a rating scale 1-5 (bad to excellent). Demographic data and results were evaluated.Results: Of the 580 patients, 389 (67.1%) were female, 191 (32.9%) were male. Total 383 were adults, parents were interviewed in pediatric group. ?Providing quick services for urgent health problems? is evaluated worst with 2.13 ± 1.32. ?The doctor listened to me? was the best with 4.45 ± 0,71. Seven EUROPEP domains improved after family medicine practice in Samsun. These domains were ?explanation of the tests and therapies?, ?appointment system?, ?getting through to the doctor and practice on the telephone?, ?providing quick services for urgent health problems?, ?appropriate referral to specialist? and ?waiting time?. Means of five domain scores decreased after the project: ?Interest in your personal situation?, ?involving you in decisions about your medical care?, ?listening to you?, ?keeping your records and data confidential?, ?thoroughness?.
