Abstract
Çalışmaya Ondokuz Mayıs Üniversitesi Tıp Fakültesi İç Hastalıkları Nefroloji servisinde Mart 2012-Haziran 2014 tarihleri arasında yatan, enfeksiyon tanısı almış, 18 yaş ve üstü, kronik böbrek hastalığı olan 72 olgu dahil edilmiştir. Elde edilen verilerden hastaların kültürde üreme olması ve kültür üreme olamaması durumuna göre CRP ve PCT değerlerinin tanı koymada etkinliği değerlendirilmiştir. Çalışmaya dahil edilen 72 hastanın 38'i erkek olup tüm hastaların yaş ortalaması 63 ± 15'ti (20- 90). Hastalarda enfeksiyon parametresi olarak PCT ve CRP değerlerine bakıldı, aynı zamanda kültürde üreme durumları değerlendirildi. Tedavi öncesi ve sonrası CRP ile PCT farkının kültürde üreme durumunu öngörmede tanısal karar verdirici özellikleri ROC eğrisi analiziyle incelendi. Belirlenen kesme değerlerine göre sensitivite, spesifisite, pozitif ve negatif prediktif değerler hesaplandı. Çalışmamızda elde edilen veriler ile tedavi sonrasında CRP değerindeki düşüşün kültürde üremeyi öngörmede etkin olduğu sonucuna ulaşıldı. Ancak PCT değerindeki düşüş kültürde üremeyi öngörmede etkin değildi.
72 cases at and over the age of 18 with chronic kidney disease who were diagnosed with infection and who were inpatients at Ondokuz Mayıs University, Faculty of Medicine, Internal Diseases, Nephrology Service between March 2012 and January 2014 were included in the study. The records of the patients were taken from the database of the hospital. Form the data obtained, the efficiency of CRP and PCT values in diagnosis were assessed according to the presence or absence of proliferation in patients' cultures. 38 of the 72 patients included in the study were men and the average age of the patients was 63 ± 15 (20- 90). As the infection parameter, the patients' PCT and CRP values were checked and the presence of proliferation was assessed in the culture. The diagnostic decision making characteristics of the difference between pre treatment and post treatment CRP and PCT in predicting the proliferation in culture were assessed with ROC curve analysis. Sensitivity, specificity, positive and negative predicative values were calculated according to the cut-off values determined. According to the data obtained as a result of our study, it was concluded that the decrease in CRP values was effective in predicting culture proliferation. However, the decrease in PCT values was not effective in predicting culture proliferation
