Abstract
Amaç: Tezde amacımız infertilite tanısı konulan ve etiyolojisi açıklanamayan hastalarda sperm dna fragmantasyonu (SDF), DNA hasarı ve protamin oranın belirlenmesidir. Ayrıca, SDF, DNA hasarı ve protamin oranını sperm parametreleri ile karşılaştırması amaçlandı. Materyal ve Metot: Çalışmaya toplam 33 idiyopatik infertil hasta ve 10 fertil erkek kontrol grubu olarak dahil edildi. Semen parametreleri Dünya sağlık örgütü (World Health Organisation, WHO) kriterlerine göre değerlendirildi. Hastaların ve kontrollerin sperm protamin seviyeleri anilin mavisiyle boyama ile analiz edildi. SDF belirlemek için TUNEL testi, sperm DNA hasarını ölçmek amacıyla ELISA testi uygulandı. Değerlendirilen veriler semen parametreleriyle kıyaslandı. Bulgular: Protamin seviyelerinin artışının fertil grup ve infertil grup arasında anlamlı bir ilişki göstermiştir (p<0,0001). TUNEL testi sonucunda SDF ölçümü yapılan infertil ve fertil bireyler arasında istatiksel olarak anlamlı bir fark bulunmadı (p= 0,829). İnfertil bireyler ve fertil bireyler arasında DNA hasarı değerleri bakımından bir fark bulunamadı (p=0.119). SDF ve DNA hasarı ile sperm parametreleri değerlendirildiğinde aralarında ilişki belirlenemedi. Histon yüzdesinin artışı ile sperm parametreleri değerlendirildiğinde yaş (p=0,01), motilite (p=0,0006), konsantrasyon (p <0,0001) ve sperm morfolojisiyle (p <0,0001) ilişki saptanmadı. DNA hasarı ve sperm parametreleri arasında bir ilişki bulunması (p>0,05). Sonuç: Bu tez çalışması, infertil ve fertil erkeklerin protamin oranlarının farklılık gösterdiğini ortaya koydu hipotezimizi destekler nitelikte bulundu. Ancak, sperm DNA hasarı ve fragmantasyonunun infertilite ile ilişkisini tam olarak ortaya koymak için çalışmadaki hasta sayısının artırılması ve daha kapsamlı bir çalışma yapılması yararlı olacaktır. CHARACTERIZATION OF SPERM DNA FRAGMENTATION AND DAMAGE, PROTAMINE RATIO AND THEIR RELATIONSHIP TO SEMEN PARAMETERS Aim: The aim of this thesis is evaluating sperm DNA fragmentation, sperm DNA damage protamine ratio and comparing these results in idiyopathic oligoastenoteratozoospermic (OAT) infertile patients and normozoospermic patients who are not certainly diagnosed with infertility and fertile men. In addition, we aimed to compare DNA fragmentation, sperm DNA damage, protamine deficiency and sperm parameters. Material and Method: Thirty three idiopathic infertile patients and 10 fertile men was included as control group. Semen parameters was evaluated acording to World Health Organisation criteria. TUNEL assay ELISA analysis and aniline blue staining methods were used to investigate sperm DNA fragmentation, DNA damage and protamination, respectively. These data were compared with semen parameters. Results: Protamine analysis demonstrated a significant association between infertile patients and fertile men (p<0.0001). There was no corelation in results of TUNEL test between infertile men and fertile men (p=0.829). Sperm DNA damage results demonstrated no association (p=0.119). There was no corelation between sperm DNA fragmentation and sperm DNA damage. A significant assosiation was found among increase in histone ration and age (p=0.01) and semen parameters, motility (p=0.0006) and sperm morphology (p<0.0001). No correlation was found between sperm DNA damage and sperm parameters (p>0.05). Conclusion: The thesis demonstrated discrepancy in protamine levels of infertile patients and fertile controls and support our hypothesis. To understand better the relationship between sperm DNA fragmentation and sperm DNA protamination in male infertility, additional studies are required.
