Publication: Behçet Hastalarında Optik Koherens Tomografi Anjiografi ile Arka Segment Vasküler Yapılarının Değerlendirilmesi
Abstract
ÖZET Amaç: Behçet üveitli (BÜ) gözlerde optik koherens tomografi anjiyografi (OKTA) bulgularını tanımlamak ve bu bulguları, non-oküler Behçet (NOB) grubu ve sağlıklı kontrol grubu (SKG) ile karşılaştırmaktır. Gereç-Yöntem: Ondokuz Mayıs Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı'nda Ekim 2023 ve Şubat 2024 tarihleri arasında klinik olarak inaktif Oküler Behçet (OB), NOB hastaları ve SKG katılımcıları OKTA (DRI OCT-1 Triton, Topcon, Tokyo, Japonya) ve Spektral Domain OKT (Heidelberg Spectralis) kullanılarak değerlendirildi. OKT ile santral makula kalınlığı, subfoveal koroid kalınlığı, retina segmentasyon analizi ile ganglion hücre tabakası, iç pleksiform tabaka, iç nükleer tabaka, iç retinal tabaka ve dış retinal tabaka kalınlıkları ölçüldü. OKTA ile 3x3 mm maküla, 4,5x4,5 mm optik disk taramaları yapıldı. Görüntülerde cihazın yazılımı (ImageNet 6; DRI OCT Triton, Topcon Corporation) kullanılarak damar yoğunlukları (%) ölçüldü. Yüzeysel kapiller pleksus ve derin kapiller pleksusta foveal avasküler zon (FAZ) alanı, FAZ perimetre ve FAZ daireselliği de cihazın kendi yazılımında olan görüntü analiz programı ile değerlendirildi. Bulgular: Çalışmamıza 35 inaktif OB hastasının 62 gözü, 31 NOB hastasının 61 gözü ve 31 SKG katılımcısının 62 gözü dahil edildi. Snellen eşeli ile ölçülen en iyi düzeltilmiş görme keskinliği (EİDGK); OB, NOB ve SKG sırası ile 0,00 (1,30-0,00), 0,00 (0,5-0,00) ve 0,00 (0,00 -0,00) LogMAR olarak ölçüldü. Yapılan çoklu karşılaştırmaya göre kontrol grubunun santral ve parafoveal ortalama VD değeri OB ve NOB grubuna göre daha yüksekti. OB ve NOB grubunda FAZ alanı, FAZ perimetre değerleri kontrol grubundan daha yüksekti. FAZ daireselliği değerleri gruplar arasında istatiksel anlamlı farklılık göstermiyordu. Sonuç: Behçet Hastalığı'nda OKTA, arka segment yapılarının anatomik ve vasküler yapıların değerlendirilmesinde önemli bilgiler sağlar.
ABSTRACT Purpose: To describe optical coherence tomography angiography (OCTA) findings in eyes with Behçet's uveitis (BU) and to compare these findings with the non-ocular Behçet's disease (NOB) group and the healthy control group (HCG). Material and Methods: Clinically inactive Ocular Behçet's (OB) patients, NOB patients, and HCG participants were examined using OKTA (DRI OCT 1 Triton, Topcon, Tokyo, Japan) and Spectral Domain OCT (Spectralis, Heidelberg, Germany) in Ondokuz Mayıs University, Ophthalmology Department between October 2023 and February 2024. Central macular thickness and subfoveal choroidal thickness were measured with OCT. Retinal segmentation analysis was performed, and the thickness of the ganglion cell layer, inner plexiform layer, inner nuclear layer, inner retinal layer, and outer retinal layer were determined. Optic Nerve (ONH) 4.5x4.5 mm and macula 3x3 mm angiography scans were obtained by OCTA. Vessel densities (%) were measured using the device's software (ImageNet 6; DRI OCT Triton, Topcon Corporation). The foveal avascular zone (FAZ) area, FAZ perimeter, and FAZ circularity in the superficial and deep capillary plexus were also evaluated. Results: Our study included 62 eyes of 35 inactive OB patients, 61 eyes of 31 NOB patients, and 62 eyes of 31 HCG participants. The best corrected visual acuity (BCVA) measured by the Snellen chart was 0.00 (1.30-0.00), 0.00 (0.5-0.00), and 0.00 (0.00-0.00) LogMar in OB, NOB and HCG respectively. Multiple comparisons showed that the control group's central and parafoveal mean VD values were higher than the OB and NOB groups. FAZ area and FAZ perimeter values in the OB and NOB groups were higher than the HCG. FAZ circularity values did not show statistically significant differences between the groups. Conclusion: OCTA provides important information in evaluating posterior segment anatomical and vascular structures in Behçet's patients.
