Abstract
ÖZET MULTİPL SKLEROZLU KADIN HASTALARDA ALT ÜRİNER SİSTEM SEMPTOMLARININ VE İNKONTİNANSIN YAŞAM KALİTESİ, SOSYAL İZOLASYON, YALNIZLIK VE YALNIZLIK TERCİHİ İLE İLİŞKİSİNİN İNCELENMESİ Berna ÖZBEY EKER Ondokuz Mayıs Üniversitesi Lisansüstü Eğitim Enstitüsü Hemşirelik Ana Bilim Dalı Yüksek Lisans,Temmuz/2024 Danışman:Dr. Öğr. Üyesi Kübra YENİ Amaç: Tanımlayıcı ve kesitsel nitelikte yapılan çalışmanın amacı Multipl Skleroz'lu (MS) kadın hastalarda alt üriner semptomlarının ve inkontinansın yaşam kalitesi, sosyal izolasyon, yalnızlık ve yalnızlık tercihi ile ilişkisini incelemektir. Materyal ve Metot: Araştırmanın evrenini bir üniversite hastanesinin nöroloji polikliniğine kayıtlı, dahil edilme kriterlerini sağlayan 170 MS'li kadın hasta oluşturmuştur. Araştırma verilerinin toplanması için hasta bilgi formu, Bristol Kadın Alt Üriner Sistem Semptom Ölçeği, İnkontinans Yaşam Kalitesi Ölçeği, Sosyal İzolasyon, Yalnızlık ve Yalnızlık Tercihi Ölçeği kullanıldı. Veriler Aralık 2022- Haziran 2023 tarihleri arasında yüz yüze veri toplama yöntemi ile elde edildi. Verilerin analizinde sayı, yüzdelik, t testi, korelasyon analizi ve yol analizi kullanıldı. Bulgular: Araştırmaya dahil edilen toplam 170 kadın hastanın yaş ortalaması 40.48 (±12.42)idi. Hastaların büyük çoğunluğunun evli (%68.2) ve ev hanımı olduğu (%52.9) saptandı. Hastaların çoğu üniversite ve üzeri (%37.6) öğretim düzeyinde eğitime sahipti. Bristol Alt Üriner Sistem Semptom Ölçeği'ne ait total puan ortalaması 20.77±12.47 idi. Ölçekten elde edilen verilere göre hastaların büyük bir kısmının (%92,9) üriner inkontinansının olduğu, bu hastaların da en sık sıkışma (urgency) tip üriner inkontinansı olduğu (%84,1) saptanmıştır. Alt üriner sistem sorunları arttıkça hastaların yalnızlık (p=0,001) ve sosyal izolasyon düzeyleri (p<0,001) artmakta idi. Ayrıca alt üriner sistem sorunları arttıkça hastaların yaşam kalitesi kötüleşmekte idi. Sonuç: MS'li kadın hastalarda alt üriner sistem sorunları görülme sıklığının oldukça yüksek olduğu, üriner inkontinans tiplerinden özellikle sıkışma (urgency) tipi üriner inkontinans görülme sıklığının daha yüksek olduğu, alt üriner sistem sorunu görülme sıklığı arttıkça hastaların yalnızlık ve sosyal izolasyon düzeylerinin arttığı ve yaşam kalitesinin olumsuz etkilendiği saptanmıştır. Prof. Dr.Kübra YENİ Objective: The aim of this descriptive and cross-sectional study was to examine the relationship of lower urinary symptoms and incontinence with quality of life, social isolation, loneliness and loneliness preference in female patients with Multiple Sclerosis (MS). Material and Method: The population of the study consisted of 170 female patients with MS registered in the neurology outpatient clinic of a university hospital who met the inclusion criteria. Patient information form, Bristol Female Lower Urinary Tract Symptom Scale, Incontinence Quality of Life Scale, Social Isolation, Loneliness and Loneliness Preference Scale were used to collect the data. Data were obtained by face-to-face data collection method between December 2022 and June 2023. Number, percentage, t test, correlation analysis and path analysis were used to analyze the data. Results: The mean age of the 170 female patients included in the study was 40.48 years (±12.42). The majority of the patients were married (68.2%) and housewives (52.9%). Most of the patients had university education or higher (37.6%). The mean total score of the Bristol Lower Urinary System Symptom Scale was 20.77±12.47. According to the data obtained from the scale, it was found that most of the patients (92.9%) had urinary incontinence, and the most common type of urinary incontinence was urgency type urinary incontinence (84.1%). Patients' loneliness (p=0.001) and social isolation levels (p<0.001) increased as lower urinary tract problems increased. In addition, the quality of life of the patients worsened as lower urinary tract problems increased. Conclusion: It was found that the incidence of lower urinary tract problems in female patients with Multiple Sclerosis was quite high, the incidence of urinary incontinence types, especially urgency type urinary incontinence, was higher, the loneliness and social isolation levels of the patients increased as the incidence of lower urinary tract problems increased and the quality of life was negatively affected.
