Acil Hemşirelerinde Merhamet Yorgunluğunu Etkileyen Faktörler

Abstract

Amaç: Bu araştırma acil servis hemşirelerinde merhamet yorgunluğunu etkileyen faktörleri belirlemek amacıyla tanımlayıcı, ilişki arayıcı ve metodolojik bir çalışma olarak yürütülmüştür. Materyal ve Metot: Araştırmanın örneklemini 01.11.2016-01.08.2017 tarihleri arasında Samsun Kamu Hastaneleri Birliği Genel Müdürlüğüne bağlı 17 hastanenin acil servisi ile Ondokuz Mayıs Üniversitesi Sağlık Uygulama ve Araştırma Merkezi acil servisinde çalışmakta olan toplam 253 hemşire oluşturmuştur. Veriler araştırmacı tarafından literatür doğrultusunda hazırlanan hemşireleri tanıtıcı bilgi formu, Merhamet Doyumu ve Merhamet Yorgunluğu Ölçeği, Minnesota İş Doyum Ölçeği ve Maslach Tükenmişlik Ölçeği kullanılarak toplanmıştır. Nitel verilerin toplanmasında ise yarı yapılandırılmış görüşme formu kullanılmıştır. Bulgular: Bu çalışmada Merhamet Doyumu ve Merhamet Yorgunluğu Ölçeği'nin geçerlik ve güvenirlik çalışmaları yürütülmüştür. 21 madde ve üç faktör ile doğrulanan ölçeğin güvenirlik seviyesi 0,728 olarak belirlenmiştir. Merhamet Doyumu ve Merhamet Yorgunluğu Ölçeği'nin Merhamet Doyumu, Tükenmişlik ve Sekonder Travmatik Stres alt boyutlarının puan ortalamasının sırasıyla 25,39±4,85, 14,45±3,80, 21,96±5,58 olduğu saptanmıştır. Acil hemşireleri ile yapılan odak grup görüşmelerinde elde edilen veriler doğrultusunda merhamet yorgunluğu kavramının bilinirliği, merhamet yorgunluğuna neden olan faktörler, merhamet yorgunluğunun belirtileri ve merhamet yorgunluğunu önleme stratejileri olmak üzere 4 tema belirlenmiştir. Sonuç: Merhamet Doyumu ve Merhamet Yorgunluğu Ölçeği'nin ülkemizde hemşirelere uygulanabilecek geçerli ve güvenilir bir araç olduğu belirlenmiştir. Çalışma yılı, mesleği bırakmayı isteme durumu, mesleğini ekonomik olarak tatmin edici bulma durumu gibi özeliklerin merhamet yorgunluğunu etkilediği bulunmuştur. Aim: This study has been conducted as a descriptive, relational and methodological study to determine the factors affecting the compassion fatigue in emergency nurses. Material and Method: The population of the study consists of a total of 253 nurses who were working in the emergency services of 17 hospitals affiliated to the General Directorate of Samsun Public Hospitals Union and the emergency service of Ondokuz Mayis University Health Application and Research Center between the dates 01.11.2016 and 01.08.2017. Data were collected by using the introductory information form prepared by the researcher in accordance with the literature, Compassion Satisfaction and Compassion Fatigue Scale, Minnesota Job Satisfaction Scale and Maslach Burnout Scale. Semi-structured interview form was used to collect qualitative data. Findings: In this study, the validity and reliability studies of the Compassion Satisfaction and Compassion Fatigue Scale (Version 5) were conducted. The reliability level of the scale confirmed by 21 items and three factors was determined as 0,728. It was determined that the mean score Compassion-Satisfaction, Burnout and Secondary Traumatic Stress sub-dimensions of the Compassion Satisfaction and Compassion Fatigue Scale were 25.39 ± 4.85, 14.45 ± 3.80, and 21.96 ± 5.58 respectively. In line with the data obtained from focus group interviews with emergency nurses 4 themes were determined including recognition of compassion fatigue, factors causing compassion fatigue, signs of compassion fatigue and strategies to prevent compassion fatigue. Result: It was designated that the Compassion Satisfaction and Compassion Fatigue Scale was a valid and reliable tool that could be applied to nurses in our country. It was found that features such as working year, request to leave the profession, finding the occupation economically satisfactory affect the compassion fatigue.
