Koah Hastalarına Özgü Yaşam Kalitesi Ölçeğinin Türkçe Versiyonunun Geçerlik ve Güvenirlik Çalışması

Abstract

Bu araştırma kronik obstrüktif akciğer hastalarının yaşam kalitesini ölçmek amacıyla Ninot ve ark. tarafından geliştirilmiş olan 'KOAH Hastalarına Özgü Yaşam Kalitesi Ölçeği'nin (COPD-Specific HRQoL Questionnaire-VQ11)' Türkçe versiyonunun geçerlik ve güvenirliğini saptamak amacıyla yapıldı. Bu araştırma 15 Şubat-30 Mayıs 2024 tarihleri arasında, Afşin Devlet Hastanesi ve Elbistan Devlet Hastanesi dahili kliniklerinde yatarak tedavi gören ve poliklinik hizmeti alan KOAH hastaları ile yüz yüze görüşme yöntemi ile gerçekleştirildi. Metodojik ve tanımlayıcı niteliğinde olan bu araştırma da örneklem grubunu araştırmaya katılmaya gönüllü olan, 18 yaş üzeri, KOAH tanısı kesinleşmiş ve en az 3 aydır KOAH tanısı olan, bilinci açık ve iletişim kurabilen 200 hasta oluşturdu. Ölçeğin dil ve kapsam geçerliği için 2 uzman hekim ve 6 öğretim üyesi hemşire tarafından toplam 8 kişiden uzman görüşü alındı ve ölçeğin KGİ (kapsam geçerlik indeksi) 0,87 olarak hesaplandı. Toplam 11 madde ve 3 alt boyutta oluşan ölçeğin birinci düzey doğrulayıcı faktör analizi sonucun incelendiğinde model uyum indeksleri CMIN/DF (114,993 / 39)=2,949, GFI=0,907, IFI=0,974, TLI=0,963, CFI=0,974, RMSEA=0,099, SRMR=0,029 olarak elde edilmiştir. RMSEA dışındaki model uyum indekslerinin tamamı kabul edilebilir sınırlar içerisinde elde edilmiştir. Maddelere ait yol katsayılarının tamamı istatistiksel olarak anlamlı bulunmuştur (p<0,001). Ölçeğin Cronbach alfa değeri düzeyi oldukça yüksek (0,971) bulundu. Ölçek ve alt boyutlarının zamana göre değişmezliğini değerlendirmek için test-tekrar test yapıldı, her iki uygulama arasında anlamlı korelasyon saptandı (p<0,001). Sonuç olarak 'KOAH Hastalarına Özgü Yaşam Kalitesi Ölçeği'nin (VQ11)' Türkçe versiyonunun Türk toplumu ve sosyokültürel özelliklerine göre kullanımının uygun olduğu saptandı.This study was conducted to determine the validity and reliability of the Turkish version of the 'COPD-Specific HRQoL Questionnaire (CODP-Specific HRQoL Questionnaire-VQ11)' developed by Ninot et al. to measure the quality of life of chronic obstructive pulmonary disease patients. This study was conducted between February 15 and May 30, 2024 with face-to-face interviews with COPD patients who received inpatient and outpatient services in the internal clinics of Afşin State Hospital and Elbistan State Hospital. In this methodological and descriptive study, the sample group consisted of 200 patients who volunteered to participate in the study, were over the age of 18, had a confirmed diagnosis of COPD and had been diagnosed with COPD for at least 3 months, were conscious and able to communicate. For the language and content validity of the scale, expert opinion was obtained from a total of 8 expert, 2 specialist physicians and 6 academician nurses, and the CGI (content validity index) of the scale was calculated as 0.87. When the results of the first level confirmatory factor analysis of the scale consisting of 11 items and 3 sub-dimensions were examined, the model fit indices were CMIN/DF (114,993 / 39)=2,949, GFI=0,907, IFI=0,974, TLI=0,963, CFI=0,974, RMSEA=0,099, SRMR=0,029. All of the model fit indices except RMSEA were obtained within acceptable limits. In addition, all path coefficients of the items were statistically significant (p<0.001). The Cronbach's alpha of the scale was found to be quite high (0.971). Test-retest was performed to evaluate the invariance of the scale and its sub-dimensions over time, and a significant correlation was found between both applications (p<0.001). In conclusion, it was determined that the Turkish version of the 'Quality of Life Scale Specific to COPD Patients (VQ11)' is appropriate for use according to Turkish structure and sociocultural characteristics.
