Publication: Meme Kanserli Hastalarda Depresyon, Umutsuzluk ve Sosyal Destek Arasındaki İlişki
Abstract
Bu araştırma meme kanserli hastalarda depresyon umutsuzluk ve sosyal destek arasındaki ilişkinin incelenmesi amacıyla tanımlayıcı ve ilişki arayıcı bir çalışma olarak yürütüldü. Araştırmanın örneklemini 10.07.2019-30.03.2020 tarihleri arasında Ondokuz Mayıs Üniversitesi Sağlık Uygulama ve Araştırma Merkezi Onkoloji Hastanesinin kemoterapi ünitesinde ayaktan tedavi gören 242 meme kanseri hastası oluşturdu. Veriler hastaların sosyo-demografik ve klinik özelliklerini belirleyici Hasta Tanıtıcı Bilgi Formu, Destekleyici Bakım Gereksinimleri Ölçeği Kısa Formu (DBGÖ-KF), Beck Umutsuzluk Ölçeği (BUÖ), Beck Depresyon Ölçeği (BDÖ) ve FACT-B Yaşam Kalitesi Ölçeği kullanılarak toplandı. Veriler IBM SPSS V23 ile analiz edildi. Normal dağılıma uygunluk Kolmogorov Smirnov ile incelendi. Değişkenler arasındaki ilişki Spearman Korelasyon analizi ile belirlendi. Ölçeklerin güvenilirlik analizi Cronbach's Alpha ile değerlendirildi. Ölçek toplam puanları ve alt boyutları ile bağımsız değişkenler arasındaki nedensellik lineer regresyon analizi ile incelendi. DBGÖ-KF toplam puan ortalaması 103.21 ± 13.56, BUÖ toplam puan ortalaması 6.18 ± 3.49, BDÖ toplam puan ortalaması 38.17 ± 6.02 ve FACT-B Yaşam Kalitesi toplam puan ortalaması 80.57 ± 17.52 olarak belirlendi. BDÖ puanı ve BUÖ puanı arasında istatistiksel olarak anlamlı, pozitif yönlü orta düzey bir ilişki belirlendi (r=0.578, p<0.001). BUÖ puanı ve DBGÖ-KF puanı arasında pozitif yönlü zayıf düzey istatistiksel olarak anlamlı bir ilişki saptandı (r=0.150, p=0.010). DBGÖ-KF puanı ve FACT-B Yaşam Kalitesi ölçek puanı arasında istatistiksel olarak anlamlı, negatif yönlü zayıf düzey bir ilişki saptandı (r=-0.165, p=0.010). Bu araştırmadan elde edilen bulgular doğrultusunda, meme kanserli hastaların depresyon ve umutsuzluk düzeyleri arasında pozitif yönlü; destekleyici bakım gereksinimleri ile yaşam kaliteleri arasında negatif yönlü bir ilişki olduğu belirlendi. Araştırma kapsamındaki hastaların depresyon ve umutsuzluk düzeyleri arttıkça destekleyici bakım gereksinimlerinin arttığı ve yaşam kalitelerinin azaldığı görüldü. This descriptive and correlational study sought to examine the correlation between depression, hopelessness and social support in patients with breast cancer. The sample of the study comprised 242 patients with breast cancer receiving ambulatory treatment in the chemotherapy unit in Ondokuz Mayıs University Health Application and Research Center Oncology Hospital between 10.07.2019 and 30.03.2020. We collected the data using the Patient Introductory Information Form determining the sociodemographic and clinical characteristics of patients, the Supportive Care Needs Scale-Short Form (SCNS-SF), the Beck Hopelessness Scale (BHS), the Beck Depression Scale (BDS) and the FACT-B Quality of Life Scale. We analyzed the data via the IBM SPSS V23. We examined the normal distribution via the Kolmogorov-Smirnov. We determined the correlation between the variables via the Spearman's Correlation analysis. We evaluated the reliability analysis of the scales via the Cronbach's Alpha. We examined the causality between the total scale and subscale scores and independent variables via the linear regression analysis. The mean total SCNS-SF score was 103.21 ± 13.56, the mean total BHS score was 6.18 ± 3.49, the mean total BDS score was 38.17 ± 6.02 and the mean total FACT-B Quality of Life Scale score was 80.57 ± 17.52. We determined a statistically significant moderate correlation between the BDS score and the BHS score in a positive direction (r=0.578, p<0.001). We determined a statistically significant weak correlation between the BHS score and the SCNS-SF score in a positive direction (r=0.150, p=0.010). We determined a statistically significant weak correlation between the SCNS-SF score and the FACT-B Quality of Life Scale score in a negative direction (r=-0.165, p=0.010). In line with the findings obtained from the study we determined a correlation between the depression and hopelessness levels of the patients with breast cancer in a positive direction and a correlation between their supportive care needs and quality of life in a negative direction. We found that as the depression and hopelessness levels of the patients within the scope of the study increased, their supportive care needs increased and quality of life decreased.
