Abstract
Türkiye'de, özellikle son 10 yılda, sağlık hizmetleri sunumunda önemli bir dönüşüm gerçekleşmiştir. Sağlık hizmetlerinin kalitesi ve çeşitliliği artmış, hizmete erişim kolaylaşmış, toplumun tamamı genel sağlık sigortası kapsamına alınmıştır. Bunun tabii sonucu olarak da kamu kesimi sağlık harcamaları bu dönemde önemli bir artış eğilimi içine girmiştir. Toplumumuzun yaşlanmasıyla birlikte sistemin kamuya finansal yükü daha da artacaktır. Bu durum sürdürülebilirlik ve etkinlik sorununu karşımıza çıkarmaktadır. Bu çalışmada, kamu sağlık harcamalarının amacına ne kadar ulaştığı, diğer yönüyle kamu sağlık hizmet sunumunun ne kadar etkin olduğu analiz edilmeye çalışılmıştır.Bu çalışmada, kamu hastanelerinin etkinliğinin ortaya konulmasında non-parametrik bir metod olan Girdiye Yönelik Veri Zarflama Analizi kullanılmıştır. Bu çalışmada karar verme değişkenlerini, büyüklükleri birbirine yakın yani birbirleriyle mukayese edilebilir, 30 devlet hastanesi oluşturmaktadır. Kullanılan girdiler; hastanedeki yatak sayısı, uzman hekim sayısı, pratisyen hekim sayısı ve döner sermaye gideridir. Çıktılar ise; hastanede toplam yatak doluluk oranı, bir hastanın ortalama kalışı (gün sayısı) ve döner sermaye geliridir.Araştırmanın ana materyalini Sağlık Bakanlığı Sağlık Hizmetleri Genel Müdürlüğü İstatistikleri ve Sağlık Bakanlığı Tekdüzen Muhasebe Sistemi kullanılarak Bakanlık yetkililerince derlenen veriler oluşturmaktadır. Veriler 2011 yılına aittir.Çalışma bulgularına göre, incelenen 30 devlet hastanesinin 14 tanesinin göreli etkin, 16 tanesinin ise etkin olmadığı anlaşılmıştır. Etkin olmayan hastanelerin yüzde 100 etkinliğe ulaşmaları için girdi miktarlarını ne kadar azaltmaları ve çıktı miktarlarını ne kadar artırmaları gerektiği hesaplanmıştır.Anahtar Kelimeler: Veri zarflama analizi, Türkiye'de devlet hastaneleri, etkinlik analizi, 2011 yılı sağlık hizmetleri verileri.
Turkey has experienced a significant transformation in the provision of healthcare services especially in the last 10 years. The quality and the variety of healthcare services have increased, access to health services has been facilitated and all citizens have been covered under universal health insurance. As a natural consequence, public health expenditures followed a rising trend in this period. The financial burden of the system to the public will rise further due to the aging of the population. This fact raises the problems of sustainability and efficiency. This study tries to analyze the extent to which public health expenditures reach their goals and how efficient the provision of public healthcare services is.A study measuring the cost effectiveness of public hospitals in Turkey has never been conducted before. This study employs the non-parametric method of Input-Oriented Data Envelopment Analysis to put forth the efficiency of public hospitals. The decision making variables of this study are composed of 30 comparable public hospitals with similar sizes. The inputs are: number of beds in the hospital, number of medical specialists, number of general practitioners and the expenditure of the revolving fund. The outputs are: total bed occupancy rate of the hospital, average stay of the patient (number of days) and the revenue of the revolving fund.The main research materials are the Statistics of the General Directorate of Healthcare Services at Ministry of Health and the data compiled by the Ministry of Health authorities via the Uniform Accounting System of Ministry of Health. The data belong to year 2011.The findings of the study reveal that 14 of the 30 public hospitals examined are relatively efficient, whereas the remaining 16 are inefficient. As regards the inefficient hospitals, how much they should decrease inputs and increase outputs so as to achieve 100 percent efficiency rate have also been calculated.Keywords: Data envelopment analysis, Public hospitals in turkey, Efficiency analysis, Healthcare services data of year 2011.
