Abstract
Amaç: Kronik şizofreni hastalarında anterior singulat girus ve kaudat nukleus metabolit seviyelerinin Manyetik Rezonans Spektroskopi (MRS) yöntemi ile karşılaştırılması, kognitif fonksiyonlarla ilişkisinin ortaya konulmasıdırYöntem: Çalışmaya hepsi sağ elini kullanan, DSM IV tanı ölçütlerine göre tanısı konulmuş 20 kronik şizofreni hastası (10 kadın, 10 erkek) ve eğitim düzeyleri, el tercihleri benzer 14 sağlıklı kontrol olgu (7 kadın, 7 erkek) alınmış; anterior singulat girus ve kaudat nukleus N-asetilaspartat (NAA), Kreatin (Cr), Kolin (Cho) metabolit düzeyleri ölçülmüş, PANSS ve nörokognitif test bataryası uygulanmıştırBulgular: Şizofreni hastalarında kontrol grubuna göre sol anterior singulat girusta NAA (p=0.007) ve Cr (p=0.023) miktarlarında azalma bulundu. Kaudat nukleusta iki grup karşılaştırıldığında anlamlı bir fark olmamasına rağmen hasta grubunda sağ ile sol arasında Cho'de (p=0.011) anlamlı bir azalma mevcuttu.Metabolitlerle kognitif performans arasında anlamlı bir korelasyon olmamakla birlikte hasta grubunda kontrollere göre kötü performans dikkati çektiSonuç: Anterior singulat girus ve kaudat nukleus limbik sistemle ilişkili önemli yapılardır ve şizofrenideki kognitif bozukluklarla ilişkili olabilir. NAA azalması; gerçek bir nöron kaybı ya da metabolik disfonksiyonu sonucu intranöronal NAA konsantrasyonunun azalmasına bağlı olarak oluşabilir. Şizofreni hastalarındaanterior singulat girusta NAA ve Cr azalması hipometabolizmayı gösterebilir. Kaudat nukleusta hasta grubunda sağ ile sol arasında Cho farklılığı kısmen fosfolipid metabolizmasındaki değişikliğe bağlı olabilir.
Objective: The aim of this study was to compare metabolite levels of anterior cingulate gyrus and nucleus caudate in patients with chronic schizophrenia by the use of Magnetic Resonance Spectroscopy (MRS) and investigate the relation of these metabolites with cognitive functionsMethod: 20 right handed patient (10 female, 10 male) whom had the diagnosis of chronic schizophrenia according to the DSM IV criteria and 14 healthy subjects (7 female, 7 male) with similar education levels and hand preferences were included in the study. N-Acetylaspartate (NAA), Creatine (Cr) and Choline (Cho) levels of anterior cingulate gyrus and caudate nucleus were measured. PANSS and neurocognitive test bataria were appliedResults: NAA (p=0.007) and Cr (p=0.023) of the left anterior cingulate gyrus was found to be lower in schizophrenia patients compared to healthy subjects. Regarding the caudate nucleus, no significant difference was found between the two groups, however, there was a significant decrease in Cho (p=0.011) between the right and left in the patient groupConclusions: Anterior cingulate gyrus and caudate nucleus are important structures related to the limbic system and may be associated with cognitive impairment in schizophrenia. The decrease in NAA in schizophrenia patients may result from the loss of neurons or a decrease in intra-neuronal NAA due tometabolic dysfunction. The decrease in NAA and Cr of the anterior cingulate gyrus may indicate hypometabolism in schizophrenia patients. The difference in Cho between the right and left caudate nucleus in the patient group may be partially due to alterations in phospholipid metabolism.
