Abstract
AmaçBu çalışmada adenotonsiller hipertrofi nedeniyle uyku bozukluğu ve oral beslenme yetersizliği şikayetleriyle adenotonsillektomi yapılan hastalarda IGF-1, IGFBP-3, Ghrelin değişimlerini göstermektir.MetodAdenotonsiller hipertrofi nedeniyle uyku solunum bozukluğu (ağzı açık uyuma, horlama, uyku düzensizliği) tanısı almış 40 çocuk (yaş ortalaması 4,85 ± 2,15) çalışmaya dahil edildi. Adenotonsillektomi yapılan hastaların preoperatif IGF-1, IGFBP-3, ghrelin değerleri ile postoperatif 6 ay sonra ki değerleri karşılaştırıldıBulgularAdenotonsillektomi sonrası tüm hastalarda serum IGF-1, IGFBP-3 değerleri anlamlı olarak yükseldi (p<0.001) ve ghrelin değerleri ise anlamlı olarak azaldı (p<0.001SonuçAdenotonsillektomi sonrası uyku kalitesinde düzelme sonucunda büyüme hormonunun diurnal salımındaki artış neticesinde IGF-1, IGFBP-3 düzeyleri anlamlı olarak artmış yine operasyon sonrası oral beslenme yetersizliğinin ortadan kalkması sonucunda ghrelin düzeylerinde anlamlı azalma gözlenmiştirDizin TerimleriIGF-1, IGFBP-3, ghrelin, adenotonsiller hipertrofi, uyku solunum bozukluğu, adenotonsillektomi.
Objective: The aim of this study was to determine the changes in insulin-like growth factor-I (IGF-1), insulin-like growth factor binding protein-3 (IGFBP-3) and ghrelin levels following adenotonsillectomy (T&A) in children with sleep disordered breathing (SDBMethods: Forty children (mean age 4.85 ± 2.15 years) clinically diagnosed with adenotonsillar hypertrophy (ATH) related SDB were enrolled. All children underwent T&A. Serum levels of IGF-1, IGFBP-3 and ghrelin were measured before and 6 months after T&AResults: Serum levels of IGF-1 and IGFBP-3 were significantly higher after T&A (p<0.001). Serum ghrelin levels showed a significant decrease after T&A (p<0.001Conclusion: Children with ATH related SDB who underwent T&A showed significant increases in IGF-1 and IGFBP-3 levels indicating an increase in diurnal growth hormone secretion as well as a significant decrease in ghrelin levels indicating an increased oral food intake in the postoperative periodKey Words: IGF-1, IGFBP-3, ghrelin, adenotonsillar hypertrophy, sleep disordered breathing, adenotonsillectomy.
