Publication: İleri Evre Kanser Hastalarının Bakım Vericilerinin Destekleyici Bakım Gereksinimleri ve Sosyal Destek Düzeyleri ile Yaşam Kaliteleri Arasındaki İlişki
Abstract
Amaç: Bu araştırma, ileri evre kanser hastalarının bakım vericilerinin destekleyici bakım gereksinimleri ve sosyal destek düzeyleri ile yaşam kaliteleri arasındaki ilişkiyi belirlemek amacıyla yürütüldü. Materyal ve Metot: Araştırma 14/03/2023-30/04/2025 tarihleri arasında bir üniversitenin onkoloji hastanesinde yürütüldü. Araştırmanın örneklemini evre 3 ve evre 4 kanser hastalarının 328 primer bakım vericisi oluşturdu. Araştırmada nicel ve nitel karma desenden yararlanıldı. Araştırmanın nicel verileri Tanıtıcı Bilgi Formu, Çok Boyutlu Algılanan Sosyal Destek Ölçeği (ÇBASDÖ), Bakım Verenlerin Destekleyici Bakım Gereksinimleri Ölçeği (BVDBGÖ) ve SF-12 Yaşam Kalitesi Ölçeği kullanılarak toplandı. Araştırmanın nitel verileri ileri evre kanser hastasının 22 bakım vericisiyle fenomonolojik görüşmeler yapılarak elde edildi. Nicel verilerin değerlendirilmesinde SPSS 25 paket programından yararlanıldı. Nitel veriler değerlendirilirken içerik analizi yapılarak temalar ve alt temalar belirlendi. Bulgular: ÇBASDÖ toplam puan ortalaması 69,02±21,21, BVDBGÖ toplam puan ortalaması 3,32±1,03, SF-12 Yaşam Kalitesi Ölçeği Fiziksel Bileşen alt boyut puan ortalaması 39,91±10,15, Mental Bileşen alt boyut puan ortalaması 32,19±9,14 olarak belirlendi. SF-12 Yaşam Kalitesi Ölçeği Fiziksel Bileşen alt boyut puanı ile BVDBGÖ toplam ve alt boyutları arasında negatif yönlü düşük ile orta düzeyde ilişki saptandı (p<0.05). SF-12 Yaşam Kalitesi Ölçeği Mental Bileşen alt boyut puanı ile BVDBGÖ toplam ile Psikolojik ve Emosyonel İhtiyaçlar, İş ve Sosyal İhtiyaçlar, İletişim ve Aile İhtiyaçları alt boyut puanları arasında negatif yönlü ve düşük düzeyde ilişki belirlendi (p<0.05). SF-12 Yaşam Kalitesi Ölçeği Mental Bileşen alt boyut puanı ile ÇBASDÖ toplam ve alt boyut puanları arasında pozitif yönlü ve orta düzey bir ilişki bulundu (p<0.05). Nitel verilerin analizi sonucunda iki tema ve dokuz alt tema belirlendi. Sonuç: Bu araştırmada, ileri evre kanser hastalarının bakım vericilerinin destekleyici bakım gereksinimleri ve algıladıkları sosyal destek düzeylerinin yaşam kalitelerini etkileyen önemli yordayıcılar olduğu belirlendi. Objective: This study aimed to examine the relationship between the supportive care needs, levels of perceived social support, and quality of life among caregivers of patients with advanced-stage cancer. Materials and Methods: The study was conducted between March 14, 2023, and April 30, 2025, at the oncology hospital of a university. The sample consisted of 328 primary caregivers of patients diagnosed with stage 3 or stage 4 cancer. In the study qualitative and quantitative mixed methods design was employed. Quantitative data were collected using the Descriptive Information Form, the Multidimensional Scale of Perceived Social Support (MSPSS), the Supportive Care Needs Survey for Caregivers (SCNS-C), and the SF-12 Quality of Life Scale. Qualitative data were obtained through phenomenological interviews conducted with 22 caregivers of patients with advanced-stage cancer. Quantitative data were analyzed using the SPSS 25 statistical software package. For qualitative data, content analysis was performed to identify themes and subthemes. Findings: The mean total score of the Multidimensional Scale of Perceived Social Support (MSPSS) was 69.02±21.21. The mean total score of the Supportive Care Needs Survey for Caregivers (SCNS-C) was 3.32±1.03. The mean score for the Physical Component subscale of the SF-12 Health Survey was 39.91±10.15, and the mean score for the Mental Component subscale was 32.19±9.14. A low to moderate negative correlation was observed between the Physical Component score of the SF-12 Health Survey and both the total and subscale scores of the SCNS-C (p<0.05). Furthermore, a low negative correlation was found between the Mental Component score of the SF-12 Health Survey and the total SCNS-C score, as well as the subscales measuring Psychological and Emotional Needs, Occupational and Social Needs, and Communication and Family Needs (p<0.05). In contrast, a moderate positive correlation was identified between the Mental Component score of the SF-12 Health Survey and both the total and subscale scores of the MSPSS (p<0.05). The analysis of qualitative data yielded two main themes and nine subthemes. Conclusion: The findings of this study revealed that the supportive care needs and perceived social support levels of caregivers of patients with advanced-stage cancer significantly predicted their quality of life.
