Diyabetik Nöropatili Hastalarda Vestibüler Sistemin Değerlendirilmesi

Abstract

Amaç: Bu çalışmada amaç, periferik ve santral vestibüler sistemi değerlendiren farklı test bataryalarını birlikte kullanarak, Tip 2 diyabetli ve diyabetik nöropatili hastalarda vestibüler sistemi değerlendirmektir. Materyal ve Metot: Çalışma grubunda; diyabetik nöropatili (n=13) ve diyabetli (n=14) bireyler iki grupta incelendi. Kontrol grubu yaş eşleştirmesi yapılan sağlıklı gönüllülerden (n=13) oluştu. Tüm katılımcılara oftalmolojik ve otoskopik muayene ve odyolojik değerlendirme sonrasında vestibüler uyarılmış servikal miyojenik potansiyeller (sVEMP), video head impulse test (vHIT) ve videonistagmografi (VNG) testleri yapıldı. Bulgular: sVEMP test sonuçlarında, diyabetik nöropatili grup ve kontrol grubu arasında bilateral latans ve amplitüd ortalamaları; diyabetli grup ile kontrol grubu arasında sol kulak P1 latansı ve bilateral amplitüd ortalamaları anlamlı olarak farklı elde edildi. vHIT testi sonucunda gruplar arasında sağ lateral ve sağ anterior semisirküler kanallarda elde edilen fark anlamlı bulundu. VNG testi sonucunda gruplar arasında sadece pursuit test 0.1 Hertz sol göz kazancında anlamlı farklılık elde edildi. Sonuç: Diyabetli ve diyabetik nöropatili bireylerde periferik ve santral vestibüler sistem kaynaklı olduğu düşünülen vestibüler disfonksiyon, çalışmada elde edilen bulgular ile ortaya konuldu. Aim: The aim of this study was to evaluate the vestibular system in patients with Type 2 diabetes and diabetic neuropathy by using different test batteries that evaluate the peripheral and central vestibular system. Material and Method: In the study group; diabetic neuropathy (n = 13) and diabetes (n = 14) were examined in two groups. The control group consisted of healthy volunteers (n = 13). Cervical vestibular evoked myogenic potentials (cVEMP), video head impulse test (vHIT) and videonistagmography (VNG) tests were performed to all participants after ophthalmologic and otoscopic examination and audiological evaluation. Results: In cVEMP test results, the mean latency and amplitude between diabetic neuropathy group and control group were significantly different. P1 latency and bilateral amplitude averages were significantly different between the diabetic group and the control group. The difference between the groups in the right lateral and right anterior semicircular canals was statistically significant. As a result of VNG test, a significant difference was obtained between the groups in the pursuit test 0.1 Hertz left eye gain. Conclusion: Vestibular dysfunction, which is thought to be caused by peripheral and central vestibular system in diabetic and diabetic neuropathy patients, was revealed by the findings of the study.
