Ondokuz Mayıs Üniversitesi Tıp Fakültesi Hastanesi Acil Servisine Başvuran 65 Yaş ve Üstü Adli Nitelikteki Olguların Değerlendirilmesi; 2011-2020

Abstract

Giriş: 65 yaş ve üstü popülasyonuna ait travmalar 21. yüzyılda öne çıkan bir sağlık sorunu olmaya devam etmektedir. Bu olgulara ilk tıbbi müdahale yapılan yerler olan acil servisler; adli tıp açısından da yeterli ve kapsayıcı muayene ile gerekli bildirimin yapılması açısından önem taşımaktadır. Gereç ve Yöntem: Ondokuz Mayıs Üniversitesi Tıp Fakültesi Hastanesi Acil Servisine 01.01.2011-31.12.2020 yılları arasında, adli nitelikte bir olay sonucu gelen/getirilen 65 yaş ve üzeri olguların hastane otomasyon sistemi üzerindeki dosya ve kayıtları retrospektif olarak incelenmiştir. Çalışmamızda olgular; demografik özellikleri ile meydana gelen olaylar, yaralanan vücut bölgesi, hastanede yatış olup olmadığı, mevcut komorbiditeleri, mortalite, olay sonucu yaşamsal tehlike gelişip gelişmediği ve adli bildirim yapılıp yapılmadığı açısından değerlendirilmiştir. Bulgular: Toplam 1960 olgunun %65,8'i erkek, %34,2'si ise kadındı. Olguların %62,8'inin olay tarihinde; 65-74 yaş aralığında olduğu görüldü. En fazla başvurunun 2020 yılında olduğu saptandı. Olguların acil servise başvurmasına neden olan en sık (%36,1) olay tipi düşmedir. En sık (%36,7) yaralanan vücut bölgesi baş-boyun, en az (%5,9) yaralanan vücut bölgesi ise pelvistir. Olguların; %68,7'si hastaneye yatırılmıştır. 135 olgu olay sonucunda hayatını kaybetmiştir. Olguların %32,8'inde mevcut yaralanması yaşamsal tehlikeye neden olmuştur. Olguların %18,1'inde adli rapor düzenlenmediği ve adli bildirimde bulunulmadığı görülmüştür. Sonuç: Gerek sosyal gerekse de iş hayatında daha fazla rol almaya başlayan yaşlı popülasyonunun; travma riski artmaktadır. İlerleyen yaş ve mevcut komorbiditeler, gençlere kıyasla yaşlılarda travmanın daha ciddi sonuçlanmasına yol açmaktadır. Hekimler özellikle kayıtlarda yetersizlik ve istismar/ihmal şüphesi açısından ayrıntılı muayene yapma konusunda gerekli dikkat ve özeni göstermelidir.

Aim: Traumas belonging to the population aged 65 and over continue to be a prominent health problem in the 21st century. Emergency services, which are the first medical interventions to these cases, is also important in terms of forensic medicine in terms of making the necessary notification with adequate and comprehensive examination. Methods: The files and records on the hospital automation system of the cases aged 65 and over who came to the Emergency Department of Ondokuz Mayıs University Faculty of Medicine between 01.01.2011 and 31.12.2020 as a result of a forensic event were retrospectively examined. The cases in our study; were evaluated in terms of demographic characteristics, events that occurred, injured body area, hospitalization and length of stay, existing comorbidities, mortality, whether a life threatening situation developed as a result of the event, and whether a forensic report was made. Results: Of the total 1960 cases, 65,8% of the cases were male and 34,2% were female. At the date of the event, 62,8% of the cases were in the 65-74 age range. The highest number of applications were made in 2020. The most (36,1%) common type of event was falling. The most (36,7%) frequently injured body area is the head and neck region, and the least (5,9%) is the pelvis. 68,7% of the cases were hospitalized. 135 cases died. In 32,8% of the cases, the injury caused a life-threatening situation. In 18,1% of the cases, a forensic report was not made Conclusion: The elderly population, which has begun to take a greater role in both social and business life; increased risk with trauma. Advancing age and existing comorbidities lead to a more serious outcome of trauma in the elderly compared to the young. Physicians should be cautious about performing a detailed examination, especially in terms of inadequacy in the records and suspicion of abuse/negligence
