Gastroözefagial reflü (GÖR) hastalarında ağız bulgularının sağlıklı bireyler ile karşılaştırılması
Özet
Amaç: Gastroözefagial Reflü (GÖR) ; gastrik içeriklerin özefagus içine istemsiz geçişidir. Bazı hastalardaki asit hareketi kroniktir, tedavi gerektirir ve Gastroözefagial Reflü hastalığı (GÖRH) olarak tanımlanır. Dental erozyon klinik terimi; diş sert dokularının patolojik, kronik, lokalize ve ağrısız kaybının fiziksel sonucunu tanımlamakta kullanılır. Hastalarda estetik, fonksiyonel veya restoratif problemlere neden olabilir. Diş sert dokuları, bakteri içermeksizin kimyasal olarak asit tarafından diş yüzeyinden uzaklaştırılmaktadır. GÖR hastalarında dental erozyon, kusma veya gastroözefagial reflü sonucu gastrik asidin dişlere ve ağız içine ulaşması ile oluşmaktadır. Çalışmanın amacı GOR hastalarında oral ve dental şikayetleri, oral hijyen alışkanlıklarını araştırmak ve sonuçları sağlıklı kontrol grubu ile karşılaştırmaktır. Bireyler ve Yöntem: Çalışmaya II GÖR hastası ve II sağlıklı birev katılmıştır. Her bir bireyin medikal ve dental hikayeleri alınmıştır. DM FT, DMFS, OHI ve dental erozyon anamnez formunda kaydedilmiştir. Tükürük akış oranları, pH ve tamponlarım kapasitesi değerleri incelenmiştir. Dental erozyon Smith ve Knıght'a göre sınıflandırılmıştır. Bulgular: GÖR hastalarında diş yüzeylerindeki kayıplar belirgin olarak daha yüksektir. Buna bağlı olarak, GÖR hastaları kontrol grubuna göre daha az dişe sahiptir. Gruplar arasında uyarılmamış tükürük akış oranı ve pil değerlerinde istatistiksel olarak anlamlı bir fark yoktur. GÖR hastalarında uyarılmış tükürük akış oranı değerleri kontrol grubuna göre daha düşüktür ve fark istatistiksel olarak anlamlıdır. Deney ve kontrol grupları arasında tamponlama kapasitesi bakımından anlamlı bir fark bulunamamıştır. Sonuç: Ağız diş sağlığını koruma amaçlı programlarda çürük riskinin belirlenmesi yanında erozyon riski de tespit edilmelidir. Aim: Gastro-oesophageal Reflux (GOR) is the involuntary passage of gastric contents into the esophagus. In some patients the acid movement becomes chronic, painful and requires treatment and is termed Gastro-oesophageal Reflux Disease (GORD). The clinical term dental erosion is used to describe the physical result of a pathologic, chronic, localized, painless loss of dental hard tissue chemically etched away from the tooth surface by acid without bacterial involvement. It may cause aesthetic, functional or restorative problems in patients. Dental erosion in GOR patients is caused by gastric acid reaching the oral cavity and the teeth as a result of vomiting or gastroesophageal reflux. The aim of the present study was to investigate oral and dental symptoms, oral hygiene habits GOR patients and to compare the results with healthy controls. Subjects and Method: II subjects with GORD and II systematically healthy subjects participated in the presen! study. The medical and dental histories of each subject were verified. The subjects were also interviewed on their oral hygiene habits. DMFT, DMFS, OHI and dental erosion were recorded in questionnaire form. The values for salivary flow rate, pli and buffer capacity were investigated. Dental erosion were classified according to Smith and Knight. Results: The missing surfaces were significantly higher in GOR patients. As a result, the patients with GOR had less teeth than control group. There were no statistically significant difference in unstimulated salivary flow rate and pH values between the groups. In GOR patients stimulated salivary flow rate values were lower than control group and the difference was statistically significant. There were no statistically significant difference in buffering capacity values between groups. Conclusion: If our aim is to prevent oral and dental health, erosion risk should also be assessed like caries risk.
