Konjenital glokomun primer tedavisinde uygulanan cerrahi yöntemlerin karşılaştırılması

Abstract

AMAÇ: Konjenital glokom, primer tedavisi cerrahi olan nadir, fakat önemli görme kayıplarına yol açan bir hastalıktır. Goniotomi, trabekülotomi, trabekülektomi veya trabekülotomi ve trabekülektomi (kombine cerrahi) gibi değişik cerrahi yöntemler primer tedavi yöntemi olarak önerilmektedir. Bu çalışmada, Ondokuz Mayıs Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalında primer konjenital glokom tedavisi için uygulanan cerrahi yöntemlerin sonuçları karşılaştırıldı. GEREÇ VE YÖNTEM: Primer konjenital glokom tanısı ile öpere edilen 19 hastanın 36 gözünün kayıtları retrospektif olarak değerlendirildi. BULGULAR: Hastaların 9'u kız, 10'u erkek olup, son kontrol muayenesinde yaş ortalamaları 7.051.75 yıl idi. 36 gözün 8'inde trabekülotomi, 16'sında trabekülektomi ve 12'sinde kombine cerrahi uygulandığı belirlendi. Genel olarak preoperatif göziçi basıncı 33.028.12 mınHg, postoperatif 18.226.28 mmHg idi. Postoperatif göziçi basıncı düşüş miktarı istatiksel olarak anlamlı bulundu (P0.001). Postoperatif takip süresi genel olarak ortalama 3.581.37 yıl idi. Postoperatif erken dönemde (ilk 1 yıl) cerrahi başarı oranları trabekülotomide %62.5, trabekülektomide %44.75, kombine cerrahide %91.6 olarak tesbit edildi. TARTIŞMA: Primer kombine cerrahi, konjenital glokomlu olgularda ideal bir cerrahi yöntem olarak değerlendirildi.

PURPOSE: Congenital glaucoma is a rare disease which in treated primarly by surgical methods may lead to significant visual loss. Different kinds of surgical methods such as trabeculotomy, trabeculectomy, combined surgery are recommended as primary surgical treatment for congenital glaucoma. In this study, the results of surgical methods which have been performed for treatment of congenital glaucoma at Department of Ophthalmology, Ondokuz May University Faculty of Medicine, were compared with each other. MATERIAL and METHODS: The medical records of 36 eyes of 19 patients with congenital glaucoma who had undergone operation were reviwed, retrospectively. RESULTS: Nine patients were female, and 19 were males. The mean age of the patients was 7.051.75 years at last visit examination. In 36 eyes who had underwent surgery, trabeculotomy in 8 eyes, trabeculectomy in 16 eyes, combined surgery in 12 eyes, were identified. The mean preoperative intraocular pressure (IOP) was overally 33.02±8.12 mmHg, and the mean postoperative IOP was overally 18.2216.28 mmHg. The percentage reduction of IOP was found statistically significant (PO.001). The mean of postoperative follow-up was overally 3.58±1.37 years. After one year follow-up, the rate of success of the trabeculotomy was 62.5%, trabeculectomy was 44.75 %, combined surgery was 91,6 %. CONCLUSION: The combined surgery have a higher success rate and It appears as an ideal procedure for surgical treatment for congenital glaucoma.
